Health History and Physical Examination
Hodges University does not require its students to get the COVID-19 vaccine or otherwise provide proof of vaccination for admission to the institution . However, a student may be required by their clinical or practicum site to have completed the COVID-19 vaccine series prior to starting their student practicum or student clinical experience . In addition, a clinical or practicum site may require a student to abide by certain masking and social distancing requirements . If a student fails to abide by the placement site’s policies and procedures, the student may be terminated from the site . The placement site may require certain attestation(s) and/ or proof of vaccination . Students required by their clinical site to have the COVID-19 vaccine must make every eff ort to obtain the completed vaccine series . Any student required to have the completed COVID-19 vaccine series, but fails to obtain the vaccine may not be placed at their practicum or clinical site . If a student is removed, terminated, or otherwise unable to participate at the site due to their vaccination status, their course progression and graduation may be delayed . If a student is terminated from a clinical site for their failure to follow the site’s policies and procedures, their grade may also be negatively impacted . Students should consult their health insurer for any potential costs of completing the COVID-19 vaccine . Students required to obtain the completed COVID-19 vaccine series may also be required to provide evidence or affir tion prior to beginning their experience at the placement site . Not mandatory but the CDC recommends for Healthcare workers . The Hepatitis B vaccine is a series of three doses . Complete documentation includes the dates of the fir t, second, and third doses of the vaccine OR a lab report for a positive Hepatitis B titer . For students deemed as “non-converters” or non- responders” immunization activity records must state for the hepatitis B requirement non-converters” or non-responders .

Meningococcal Vaccine
Documented series of polio immunizations . In the absence of documentation, this polio vaccination is assumed for students who attended school in the US . Students educated outside the US may be at risk unless they are vaccinated .
