TWO WEEKS PRIOR TO ARRIVAL Youth leaders and parents of individual campers must log on to their online account and enter the detailed information for each camper. Persons Authorized for Camper Check-Out: When logged into your reservation account, click on your Current Reservation and you will see the camper profile(s). Under each camper profile, there is a Persons Authorized for Camper Check-Out verification, which is required for every camper. If the camper is being picked up by someone other than the Account Holder (person who registered them), that pick up person's first/last name and contact information is required prior to Camper Check-Out on Sunday. Enter camper cabinmate requests under the camper profile below personal information. If you are registering 5 or more campers, please update your reservation status to a Group/Church status, rather than individual status. Campers registered as a group will be housed together. Only group leaders are allowed to make any changes to the group reservation. Electronic Health Forms must be submitted 2 weeks prior to arrival. Your camper must have a completed health form in order to participate in camp activities. Access and complete the Online Camper Health Form by following the steps below: 1.Log into your account 2.Click on “Health Form” under Current Reservation. 3.This will take you to a page with a Health Form attached to each camper who is registered. 4.When each step of the health form has been completed, you may sign at the bottom and hit submit. Once Health Forms are submitted, there will be a status of “pending review.” This means the Health Center will be reviewing each Health Form and will update the status to “Need files” if something is missing, or “Accepted” if the Health Form was filled out accurately. 5.Group Leaders – you will need to send the email health form link to each parent by clicking on “Email Health Forms” at top left of this same page. Fill in the parent’s email address beside each camper name and click SEND. 6.Bring any medication listed on the health form in its original prescription container with you to Camper Check-In.
