They can offer information and advice regarding non-clinical / p co-ordination, including referral and sign-posting to community-based patient clinics, in-patient care commitments and any health-related con communication between patients, their GP and the multi-disciplinary te given as a ‘first line of contact’. In Scotland, the Scottish Government have provided fundi the aim being to improve patient communication, experie SPOC teams are involved throughout the ‘cancer jour treatment, to follow- GP / primary care: treat all common medical conditions and refer patients to hospitals and other medical services for urgent / specialist care. They provide information, advice and support for people with chronic illness and long-term conditions, aiming to keep them well and in their own homes. A GP may also provide advice and treat adverse effects / complications of cancer treatment but they will need accurate and up to date information from your specialist team, about your care, to be able to do so. It can help if your team provides a plan for further management and whom the GP should contact if problems arise. GPs may be able to support cancer-related mental health problems, as are usually aware of locally-based support services, as well as working with you to improve and maintain your general physical state of health.
