2. Determine whether the material to be copied utilizes material copyrighted by another owner or publisher. Acknowledgments of other copyrighted material will appear in the title page. This material, if copied, may require permission from the original author or publisher. 3. Submit a request to duplicate the material (see Appendix G). The request should contain the following: a. The title, author and/or editor, and edition of the materials to be duplicated; b. Description of the exact material to be used, giving the amount, page numbers, chapters and, if possible, a photocopy of the material; c. The number of copies to be made; g. A description of the type of reprint that will be utilized (ditto, photocopy, offset, typeset). The request should be sent, together with a self-addressed return envelope, to the permissions department of the publisher. If the address of the publisher does not appear at the front of the material, it may be readily obtained in a publication entitled The Literary Marketplace, which is available in most public libraries. d. A description of the use to be made of duplicated materials; e. Indicate the form in which the materials will be distributed; f. State whether the material is to be sold; and Because each request must be checked closely by the publisher, a request for permission should be submitted well in advance of the date that the materials will need to be copied. Request all permission for a specific project at the same time. Public Performance License Obtaining a public performance license to show a film is relatively easy and usually requires no more than a phone call or submission of a written or online form request. While fees vary, they are generally inexpensive for smaller audiences. Most licensing fees are based on a particular performance or set of performances for specified films.
