Prestige Series Voluntary Benefits
• Guaranteed issue — no health questions. • Pays all eligible expenses for accidents, as well as additional benefits up to the member’s total maximum benefit amount. • No calendar-year maximums and multiple claims can be filed each year. • Benefits payable are driven by first diagnosis (not by treatment received), in addition to any other in-force coverage, including medical. • Three categories of benefits available, allowing for the lump sum benefit to be paid three times (four with Benefit Recurrence). • Employees select plans that fit their family’s financial needs, with benefit values ranging from $5,000 to 50,000. • Pays in addition to any other disability coverage. • Benefit available up to 60% of monthly income. • Benefit set at time of application and does not change based on income. • Multiple tiered program combines Hospital Indemnity, Accident Critical Illness/ Cancer, and Well-Benefit benefit into one policy. • Benefits may be crafted to meet specific employer group needs. • Combined plans simplify offering to employees. • Pays employees a lump-sum cash benefit when they’re hospitalized, in addition to existing coverage, including medical plans. • Basic coverage includes a hospital daily benefit and waiver of premium, includes family coverage for spouses and children. • Program can be customized to meet specific employer group needs, with over 16 riders from which to choose.

Term Life
• Includes a health screen benefit and a cash reward program for positive lifestyle changes. • Payout for up to 21 health screens (once per year). • Includes an additional cash payout if diagnosed with one of six chronic conditions and insured joins a covered improvement program, third payout for completing 90 days of improvement program. • Guaranteed cash value with NO penalty for early distribution or payout. • Cash value increases at a consistent rate that never changes and is not tied to the CPI. • Accelerated benefits for terminal illness and facility care.
