NYS Justice Center Annual Report 2025

MORTALITY REVIEWS

CONCLUSION

CONCLUSION In 2025, the NYS Justice Center continued its vital mission to protect people who receive care from state programs. As reflected in this report, the agency is closing cases more efficiently, engaging more deeply, and investing in prevention to create safer systems.

2025 AND BEYOND

Through small-but-powerful adjustments to its investigations processes and engagement with fellow State agencies, the Justice Center was able to close 1,600 more cases in 2025 than the year prior. At the same time, the agency continued its work addressing systemic deficiencies in the care system, auditing 300 corrective action plans in 2025. These audits and continuous monitoring have led to immediate actions from licensing entities, improved safeguards, enhanced professional training programs, and other notable reforms. This work underscores our role not only as an oversight agency, but as a driver of system-wide quality improvement. Prevention was also a defining focus of 2025. The launch of the Prevention Collaborative group reflects a forward-looking approach to abuse and neglect prevention grounded in data, stakeholder feedback, and cross- agency partnership. By reducing administrative complexity where appropriate, improving reporting systems, and offering technical assistance to providers, the Justice Center continues to balance oversight with practical support for the workforce. The agency also deepened its commitment to individuals and families. Advocacy work increased significantly, and outreach events reached thousands of New Yorkers. Programs such as SDMC and TRAID provided tangible, life-changing support — ensuring access to medical decision-making alternatives and assistive technology that promote independence and community inclusion. The progress outlined in this report affirms the Justice Center’s core mission: to foster a service system where safety, transparency, and human dignity are non-negotiable.
