Ask Tell Ask Process of Advance Care Planning A process to guide decision-making about the health care you hope for if you do not have capacity to decide for yourself. Two components: Power of Attorney for Healthcare: The person(s) you would like to make healthcare decisions for you, if you do not have capacity to decide for yourself. Advance Directives: Guidance about the life and care you hope for in the face of specific situations, i.e., when you have advancing illness or catastrophic events.
