OPTIONAL ANNUAL WELLNESS BENEFIT RIDER* $60 paid each year per covered person under the policy for any one of the following examinations: • Annual Physical Examination • Immunization • Dental Exam • Flexible Sigmoidoscopies • Mammogram • PSA Test • Pap Smear • Ultrasounds • Eye Examination • Blood Screening Test The Policy must be in force 9 months before this benefit is payable.
