Transition Manager: Start Date: Personalized Property Management has been contracted to take on the management of the association. No doubt we both are looking forward to a smooth transition. The sooner we obtain operational documents, the better the transition can be. As soon as possible please forward the following: The contact information of the transition coordinator from your company 1 Member listing with property and billing addresses, phone numbers and email addresses, lot and tract numbers if applicable. 2 Association's federal tax ID number and State corporate ID number 3 Assessment information. If assessments are variable, please provide schedule showing rate per lot. 4 Fiscal Year End Date 5 Most recent annual disclosure mailing, including current association budget 6 If applicable, special assessment notice 7 Association governing documents including CC&Rs, Articles, Bylaws, Rules, etc.
As transition progresses, we will need the following: 8 Bank account information 9 A current financial (which may not be the final). 10 Current tax returns and audit/review documents 11 Historical files 12. Association Insurance information 13 Minutes of meetings 14
