Accidental Death Benefit Rider

Optional Rider: If the rider is selected, the applicant must have a face amount in the base policy that is equal to, or higher than, the Accidental Death Benefit Rider selected. For example: If Insured would like an Accidental Death Benefit Rider of $5,000, they must have selected a $5,000 or higher death benefit. (Rider available up to age 65.)

Rate Guide - ARC • Issue Age Rates: Final Expense rates vary by rate class and age. Consult Rate Sheet per State. • Preferred Rating (non-tobacco) Standard Rating (tobacco). • (Male/Female rating apply) * Height & weight can affect rating in all states and in all age ranges.

Product - Key Features Overview.

Ineligible Persons include anyone who: • Is currently incarcerated, is on parole or was released from prison within the last 2 years. • Is terminally ill, in a psychiatric facility, or mental incapacity and/or lacks the legal capacity or mental facility to conduct their own affairs. • Has not been a permanent US resident for at least 12 months. • Infant is younger than 6 months old. 4 Non-Forfeiture Options (In the event the policy holder is unable to afford to their premium payments) • Cash Value: policy holder can surrender their policy for full cash value. • Paid up Insurance: policy holder can request, to place policy on paid up status. Cash value is used to purchase a reduced face value, and the policy holder will never have to make another premium payment. • Automatic Premium Loan: if we stop receiving premium, the policy holder can use available cash value to take a loan out and pay premium payment. • Extended term: cash value can be used to purchase coverage for the full death benefit amount for a certain period.

• High blood pressure and diabetes is an automatic decline. • If they have type II non-insulin diabetic- rated up 6- years. • If they have high blood pressure rated up 6- years • If they have type II non-insulin diabetic and over H/W- rated up 7 years. • If, they have high blood pressure and over H/W- rated up 7 years. • Height & Weight • If over Preferred Maximum Weight limit in Height/Weight Add 7 years to age • If over Standard Maximum Weight limit in Height/Weight Table Decline. Applicants 0-49

• Medical questions on application, point of sale telephone interview, prescription drug check and MIB check. • If insurable interest is questionable, a background check may be ordered. • We must order medical records: a. If client is age 80+ b. On all additional applications, reinstatements.

Applicants 50-85

• Insulin dependent diabetes is acceptable if units don’t exceed 50 per day. • Type 1 insulin diabetes is an automatic decline in the state of Florida. • Height & Weight • If applicant’s weight is at or below the Preferred Maximum, they qualify for Preferred Rating. • If applicant’s weight is above the Preferred Maximum and at or below the Standard Maximum, they qualify for Standard Rating. • If applicant’s weight is above the Standard Maximum, the applicant is declined.
