Pursuing excellence through real-world data, effective guidelines and service improvements. IOF’s Capture the Fracture® (CTF) programme connects a global network of over 1100 Fracture Liaison Services (FLS), which identify, evaluate, treat, and monitor patients after a fracture. This represents a wealth of real-world data, invaluable for understanding and improving the quality of care worldwide. In this issue of FractureFocus, IOF interviews Prof. M.L. Brandi, chair of the CTF Governance, on the value of gathering real-world data and how analysing such data can lead to improved quality of care and policy change. Quality of care is also illustrated in this issue in our spotlight of the Going for Gold campaign, an initiative recognizing FLS excellence and encouraging
services to meet the highest standards. This campaign exemplifies how fostering excellence translates into better outcomes for patients and healthcare systems. Further topics in this issue include Colombia’s and Saudi Arabia’s new clinical practice guidelines, a new publication on fracture prevention in Taiwan, the national FracTurk initiative in Türkiye and more. SAUDI ARABIA: SAUDI OSTEOPOROSIS SOCIETY RELEASES UPDATED NATIONAL GUIDELINES
The Saudi Osteoporosis Society has released updated national guidelines to address the growing public health challenge of fragility fractures in Saudi Arabia, which have risen by over 25% from 2019 to 2023 due to an aging population. Despite the availability of safe and effective medications and improvements in treatment rates for osteoporosis in Saudi Arabia, treatment initiation remains suboptimal, with studies showing less than 10% of high-risk patients receiving effective secondary fracture prevention.

To tackle these issues, the 2023 guideline incorporated the Saudi- specific FRAX model for localised fracture risk assessment, enhanced vitamin D and calcium management

strategies, and new treatment options such as romosozumab and sequential therapy. Importantly, they also emphasise establishing Fracture Liaison Services (FLS) to prevent secondary fractures and improve patient care outcomes. While the updated guidelines have been praised for their structured, evidence-based approach and potential to improve osteoporosis care across Saudi Arabia, specific challenges remain in their implementation. Despite these hurdles, the guidelines have been welcomed by policymakers and healthcare professionals, offering hope for reduced osteoporosis- related burdens and alignment with international best practices. COLOMBIA: THE FLS MODEL RECOGNIZED IN NEW NATIONAL CLINICAL PRACTICE GUIDELINE
The new clinical practice guideline for the diagnosis and treatment of osteoporosis in Colombia marks a significant advance in secondary fracture prevention by including the Fracture Liaison Service (FLS) model. The guideline was launched during the Latin American Course on Bone and Muscle Health and the 8th Advanced Osteoporosis Course for Residents hosted by the Colombian Association of Osteoporosis and Mineral Metabolism (ACOMM).

FLS CHAMPIONS SHARE THEIR INSIGHTS GOING FOR GOLD
As the world celebrated the highest level of athletic excellence in Paris during the 2024 Summer Olympics, champions of fracture care worldwide were also performing at gold-medal levels in secondary fracture prevention and osteoporosis management. These champions are the dedicated teams behind the gold-star Fracture Liaison Services (FLS) – post fracture care services that play a critical role in preventing subsequent fractures in patients at highest risk.
To mark this campaign, top- performing FLS services were invited to share their insights about how they deliver gold-standard care. Eighteen of the 120 Gold-level FLS within the CTF network, which comprises more than 1100 FLS worldwide, shared images and inspiring quotes on behalf of their teams.

OF THE SWISS/ALPS FLS NETWORK FOURTH MEETING
On October 25th, the Swiss/Alps FLS Network convened for the 4th consecutive year. Participants shared innovations and solutions to challenges they implemented in their FLS in the past year. Guest lectures included quality improvement experience from Norway, the trauma surgeon perspective from FFN-CH, and updates from IOF and the Swiss AgO.
The Swiss FLS Network grew by an impressive 30% since the previous meeting, attesting to the ever-growing momentum for post-fracture care in Switzerland.

The Capture the Fracture® (CTF) programme had the pleasure of welcoming the first FLS from Bulgaria to the Map of Best Practice, making Bulgaria the 60 th country to the join the Network. FRACTURK DATABASE STANDARDISING OSTEOPOROSIS CARE IN TÜRKIYE
Fragility fractures are a growing public health concern in Türkiye, with significant health and financial implications. To address this, the Turkish Osteoporosis Society (TOS) launched the FracTurk Database in November 2023. This national initiative aims to standardize osteoporosis care by coordinating Fracture Liaison Services (FLS) across the country and improving treatment protocols.

Taiwan’s super-aging society is facing a projected 170% increase in fragility fractures between 2010 and 2035. A new report by leading Taiwanese experts outlines five key actions to address this challenge: prioritizing fragility fractures in healthcare with additional funding for post-fracture care and DXA diagnostic testing; expanding coverage of osteoporosis treatment to include primary prevention for at-risk individuals; increasing the deployment of FLS for better screening and treatment; promoting public education on bone health; and ensuring robust data collection to assess the burden of fractures and the cost-effectiveness of interventions.
