In a year marked by uncertainty and change, Fidelis Care remained unwavering in being Here for Your Health. This theme guided every step we took in 2025, reminding us that being present for our members means more than delivering care. It means showing up with compassion, consistency, and commitment, no matter the circumstances. From the bustling streets of New York City to the quiet corners of the North Country and stretching into Western New York, our teams demonstrated what it truly means to be here for our members and the communities that we serve. We didn’t just provide services, we built trust, fostered connection, and responded to real needs in real time. Whether through mobile health clinics, Veggies to Go produce giveaways, Unity events, or community baby showers, our presence was felt in meaningful and personal ways across the state. The year was not without its challenges. Communities faced economic pressures, health disparities, and social barriers, but Fidelis Care stood firm. We expanded our reach, deepened our partnerships, and listened more closely than ever. Our employees volunteered their time, our providers leaned into collaboration, and our organization invested in solutions that reflect the full spectrum of what health truly means. “Here for Your Health” isn’t just a slogan. It is a promise. A promise to be there when it matters most, to meet people where they are, and to support them in every aspect of their health journey. HERE FOR YOUR HEALTH LAUNCH

Fidelis Care launched its Here for Your Health (HFYH) campaign in the fall of 2025, reaffirming our commitment to the health and well-being of every New Yorker. The campaign highlights Fidelis Care’s mission to ensure individuals receive timely, accessible, and personalized care. Beautiful, sunny October days greeted HFYH launch events at the GrowNYC’s Bed-Stuy Farm and Capital Roots community gardens in Brooklyn and Albany. Fidelis Care leaders along with Marketing, Communications, Community Relations, and other staff members distributed more than 300 bags of fresh produce at accompanying Here for Your Health Veggies to Go giveaways and volunteered to help get the gardens ready for the spring planting season. More Here for Your Health launch events are planned for 2026 with other community partners statewide. Through these strategic partnerships and community-based outreach, HFYH connects New Yorkers with essential health services and resources that support physical, mental, and emotional well-being, while actively working to bridge gaps in care across communities. As part of our ongoing dedication to building healthier communities across New York State, Fidelis Care supports the HFYH campaign through a series of impactful initiatives: • Statewide Garden Partnerships: Fidelis Care has partnered with community gardens and urban agriculture programs through the New York State Department of Agriculture Community Gardens Task Force to increase access to fresh, nutritious food. These collaborations also support local wellness initiatives, reinforcing the importance of healthy living at the grassroots level. • $500,000 in HFYH Health Equity Grants: Funding will be awarded to community-based organizations and healthcare providers focused on reducing health disparities and improving outcomes in underserved populations. These grants are designed to empower local leaders and drive meaningful change in health equity. The Here for Your Health campaign also includes a focus on culturally competent and multilingual services offered at more than 30 community offices and hundreds of StreetSide RV mobile office stops across New York. Traditional and digital advertising, public relations efforts, and social media messaging will amplify the Here for Your Health campaign across the state throughout 2026.

Expanding Access to Care To better understand and address barriers to health, Fidelis Care conducted more than 295,000 Health-Related Social Needs (HRSN) Screenings Questionnaires, identifying needs related to food, housing, and healthcare access. These insights helped guide our outreach and connect members with the right resources, reinforcing our commitment to closing care gaps and promoting health equity across New York State.

CLOSING CARE GAPS & CREATING CONNECTIONS

Fidelis Care’s preventive care efforts reached thousands of New Yorkers through health initiatives like mobile mammograms, traveling dental units, and pop-up immunization clinics. In collaboration with trusted providers such as Upstate University Hospital in Syracuse and Urban Heath Plan in NYC, these services helped bridge gaps in access to prevention, screenings, and early detection, especially for underserved populations. Expanded Fidelis Care telehealth options offered flexible, remote access to care for members across urban and rural regions. Fidelis Care also announced the Center for Community Advancement in partnership with the Buffalo Urban League, marking a major milestone in health equity and local empowerment. This new center, part of the Buffalo Urban League’s revitalized headquarters, will serve as a hub for outreach, education, and community-based health initiatives. It reflects our shared vision of bringing care closer to the people who need it most, especially in historically underserved neighborhoods.

STREETSIDE RVS & COMMUNITY OFFICES

Fidelis Care’s commitment to accessible, community-based healthcare was brought to life in 2025 through our fleet of StreetSide RV mobile offices and more than 30 Community Offices across New York State. These two methods of outreach helped ensure that individuals and families could get the support they needed, whether at a local street corner, neighborhood event, or conveniently located office. The StreetSide RVs made nearly 300 appearances , drawing more than 8,100 visitors statewide. The RVs serve as mobile health hubs that bring coverage directly to communities. From Buffalo to the Bronx, these vehicles offered walk-up access to health insurance enrollment, membership renewals, and personalized guidance with no appointments required. Staffed by knowledgeable Fidelis Care representatives, the StreetSide RVs helped thousands of New Yorkers navigate their healthcare options in a comfortable, convenient setting. Meanwhile, Fidelis Care’s Community Offices continued to stand as anchors of support, taking care of the needs of more than 100,000 people statewide . The Staten Island office relocated to a more popular spot, and offices in Hempstead, Ridgewood, and Flushing underwent updates. This work helped create more modern, welcoming environments equipped with improved technology and accessibility. Locations in Utica, Poughkeepsie, and beyond remained vital resources for in-person assistance, helping members understand their benefits and connect with additional services.

Investing in Our Communities Meaningful health outcomes begin with strong, connected communities. In 2025, Fidelis Care expanded our outreach through a series of high-impact initiatives designed to promote wellness, equity, and engagement across New York State. From fresh food distribution to cultural celebrations and educational support, these programs reflect Fidelis Care’s commitment to meeting people where they are, physically, emotionally, and socially.

Diversity Through Literacy Through our Diversity Through Literacy initiative with Literacy in Community (LINC) in New York City, Fidelis Care supported programs that promote reading, cultural awareness, and educational equity. These efforts helped children and families build stronger foundations for lifelong learning and health.

Backpack Heroes Backpack Heroes provided healthy food to students in need in the Capital Region, helping them get through the school year prepared and confident. The initiative also connected families with preventive care information and healthcare enrollment support.

KEY INITIATIVES

Veggies to Go Fidelis Care hosted more than 80 Veggies to Go events statewide, distributing more than 8,000 bags of fresh produce to families in need. These events helped combat food insecurity while promoting healthy eating and connecting communities with local resources.

New York State Fair At the New York State Fair in Syracuse, Fidelis Care engaged thousands with health screenings, mobile dental services, and interactive wellness booths at the Women’s Day Health Fair. Our presence helped raise awareness about coverage options and preventive care in a family-friendly setting.

Unity Events With more than 10,000 participants , our Unity events celebrated diversity, inclusion, and community strength. These gatherings offered families access to health services, educational materials, and joyful experiences that fostered connection and trust.

UB Bulls Fidelis Care partnered with University at Buffalo Bulls Athletics to promote youth wellness and emotional resilience. Events like Wellness Wednesdays and community tournaments offered kids tools to manage stress and build healthy habits.

Rochester Public Market At the Rochester Public Market, Fidelis Care partnered with Cornell Cooperative Extension and Wilmot Cancer Center to host cooking demonstrations and wellness activities. These events empowered residents with practical tools for healthy living and disease prevention.

In 2025, Fidelis Care employees demonstrated their commitment to community service through volunteer events, contributing their time and effort to causes ranging from school readiness and food pantry support to neighborhood cleanups. Across regions, staff cultivated gardens, sorted donations, served meals, and distributed hygiene kits, bringing care and compassion directly to those in need. In addition to hands-on service, Fidelis Care invested in communities through strategic donations and gift spending, supporting local organizations with supplies, funding, and resources that helped expand their reach and impact.

Connecting Care to Real Life Fidelis Care recognizes that health is shaped by more than clinical visits. It’s also influenced by the environments where people live, work, and raise their families. In 2025, we strengthened our approach to whole-person care by identifying social needs and building networks that connect members to vital community resources.

Fidelis Care awarded $130,000 in rural health grants to organizations such as Literacy West NY and Restoration Society, Inc., which are expanding access to telehealth, transportation, and health literacy programs. These efforts are especially critical in areas where provider shortages and geographic isolation limit access to care. The behavioral health grant program awarded $149,000 to nine organizations , including the Center for Comprehensive Health Practice in Harlem and Pediatric Wellness of Northern New York in Watertown. These grants supported models that integrate behavioral health into primary care, with a focus on underserved youth and families. To address food insecurity, Fidelis Care launched a Food Is Medicine grant initiative, giving $130,000 to 10 organizations like Feedmore WNY and Nassau County Department of Health that feature programs integrating nutrition into healthcare delivery. With over 2.2 million New Yorkers affected by food insecurity, including 14% of children, these grants help local organizations connect families with healthy food and chronic disease prevention resources.

HEALTH EQUITY GRANTS

Whether through mobile outreach, community-based education, or direct service delivery, Fidelis Care’s health equity grants are helping build a healthier, more equitable New York, one partnership at a time.

Fidelis Care advanced our commitment to promoting health equity in 2025 through targeted grant programs that support providers and organizations in underserved communities across New York State. These grants, focused on maternal health, rural health, behavioral health, and food security, provided $559,000 in grants to more than 30 organizations . Each recipient plays a vital role in addressing barriers to care and improving health outcomes for vulnerable populations. From doula training programs in Buffalo to health literacy workshops in Allegany County, these initiatives reflect Fidelis Care’s belief that meaningful change begins at the community level. Among the latest recipients, nine organizations received $150,000 in maternal health grants , including Mosaic of the Southern Tier, Westchester Community Health Center, and Caribbean Women’s Health Association of Kings County. The grants assist organizations and providers that play a vital role in ensuring safe pregnancies and healthy babies, particularly for underserved, lower-income women and their families across New York.

In 2025, we expanded programs that support new and expectant parents, offering resources, education, and care in welcoming, community-based settings. These efforts reflect our belief that investing in families today builds stronger, healthier generations tomorrow.

HEALTH-RELATED SOCIAL NEEDS SCREENINGS QUESTIONNAIRES

In 2025, Fidelis Care collected more than 295,000 Health-Related Social Needs (HRSN) Screenings Questionnaires across New York, helping to identify critical non-medical factors that influence member health such as food insecurity, housing instability, transportation barriers, and access to care. These screenings were conducted at community events, offices, and through outreach programs, allowing Fidelis Care to connect individuals with tailored resources and support services. By integrating HRSN data into care coordination and provider partnerships, Fidelis Care is working to close gaps in health equity and ensure that every member receives care that reflects their full life context.

SOCIAL CARE NETWORKS

In support of New York State’s 1115 Medicaid Waiver that addresses overall quality and health outcomes, Fidelis Care partnered closely with Social Care Networks (SCNs) to address the broader needs that impact health such as housing, food access, transportation, and emotional support. By integrating social services into care coordination, SCNs help ensure that members receive holistic support beyond clinical care. These networks allow providers to identify social barriers and connect individuals with the right resources. To strengthen member engagement with SCNs, Fidelis Care launched targeted efforts to raise awareness and improve accessibility, resulting in more than 70,000 screenings . Website updates made it easier for members to find local resources and understand how SCNs can support their health journey. Social media campaigns highlighted practical tips, while partnerships with providers, community organizations, and SCNs themselves helped amplify outreach. These efforts ensured that more members could connect with the support they need, whether through digital platforms, trusted community partners, or direct referrals.

The Fidelis Care Utica Community Office opened a baby supplies closet in partnership with parent support organization Mom Starts Here. The initiative provides essential baby items including diapers, clothing, and more to families in need. Following up on the success in Utica, the Fidelis Care Southern Boulevard Community Office in the Bronx opened its own baby supplies closet, with more planned statewide.

In 2025, SCNs played a key role in reducing avoidable hospital visits and improving access to preventive services, reinforcing our commitment to health equity and whole-person care.

COMMUNITY BABY SHOWERS

Fidelis Care hosted more than 30 Community Baby Showers statewide, reaching nearly 3,000 new and expectant parents . These events offered baby supplies, health education, raffle prizes, and connections to local programs, creating joyful, supportive environments that promote maternal health and early childhood wellness. By partnering with local organizations, Fidelis Care helps ensure that new parents have access to the supplies and support they need during the critical early months of parenthood.

STRENGTHENING YOUTH MENTAL HEALTH WITH THE JED FOUNDATION

In 2025, Fidelis Care and the Centene Foundation partnered with The Jed Foundation (JED) on a $1.1 million , two-year initiative to strengthen mental health support for youth across New York State. JED works with community-based organizations to provide evidence-based best practices, technical assistance, and training programs that help protect youth mental health and prevent suicide. Through this partnership, under-resourced organizations gain access to JED’s comprehensive 18-month technical assistance program, targeted educational workshops, and custom solutions designed to equip staff and volunteers with the tools they need to support emotional well-being. In the first year, funding from Fidelis Care enabled three community-based organizations to enroll in JED’s technical assistance program and supported 10 educational workshops for six additional organizations. Building on this success, JED has partnered with America’s Promise Alliance (APA) to launch the Youth Mental Health Institute, which will advance best-in-class mental health practices for APA’s community of national youth-serving community-based organizations. FIDELIS CARE PARTNERS WITH SHATTERPROOF TO END ADDICTION STIGMA

Fidelis Care established a new partnership with the national nonprofit, Shatterproof, to support their efforts against addiction stigma. A Fidelis Care team joined thousands in New York City in October for the Shatterproof Walk to End Addiction Stigma. Other Fidelis Care teams across New York State participated in the Shatterproof Walk Your Way events. Together, Fidelis Care raised nearly $21,650 for the fight against addiction stigma . Look for more efforts in this partnership coming your way in 2026. The partnership is the latest effort in Fidelis Care’s “End the Stigma of Addiction” initiative, with the overarching goal to increase addiction awareness, offer accessible resources, and combat the stigma around addiction. This partnership with Shatterproof underlines our commitment to creating a supportive culture where everyone feels respected, heard, and empowered to reach out for help. As a company dedicated to ensuring access to quality care, Fidelis Care is proud to support Shatterproof’s ongoing commitment to create lasting change, break down stigma, and support communities affected by addiction. We recognize that addiction is a public health issue, not a personal failing. Let’s challenge stigma, support one another, and encourage open conversations about addiction and recovery.

MEDICAID ADVANTAGE PLUS (MAP) GROWTH

In 2025, Fidelis Care significantly expanded its Medicaid Advantage Plus (MAP) program, known as Wellcare Fidelis Dual Plus, to reach more eligible New Yorkers across 59 counties statewide . This growth reflects Fidelis Care’s commitment to providing coordinated, patient-centered care for individuals who qualify for both Medicare and Medicaid and require long-term support. To support product growth, Fidelis Care established a specialized MAP engagement team to spearhead efforts to help more individuals through the extensive enrollment process. Our increased MAP initiatives resulted in 150% membership growth in 2025. The MAP plan offers comprehensive benefits including home health aides, adult day healthcare, mental health services, dental care, and $0 prescription copays while helping members remain safely in their homes and communities.

ADVANCING HEALTH EQUITY THROUGH VALUE-BASED PARTNERSHIPS

Fidelis Care’s value-based partnerships are a cornerstone of our mission to deliver high-quality, equitable care to members across New York State. By aligning provider incentives with improved health outcomes and cost efficiency, Fidelis Care ensures that care is not only accessible, but also effective and sustainable. These partnerships are designed to reward providers for delivering preventive, coordinated care that keeps patients healthier and reduces unnecessary hospital visits. Through collaborations with Accountable Care Organizations, Independent Physician Associations, hospital systems, community practices, and Social Care Networks, Fidelis Care addresses the full spectrum of member needs: physical, behavioral, and social. These groups leverage Fidelis Care’s robust data tools to identify gaps in care, reduce avoidable hospital visits, and improve access to preventive services. Fidelis Care holds more than 60 unique value-based partnerships across multiple lines of business, including Medicaid, HARP, and the Essential Plan, representing over 90% of Fidelis Care’s Medicaid experience . The savings generated through these partnerships are reinvested directly into the communities that Fidelis Care serves, supporting local providers and expanding care access. As the program evolves, Fidelis Care continues to integrate health equity goals and align with New York State Department of Health initiatives. These efforts ensure that value-based care is not just a financial model, but a pathway to healthier, more resilient communities.

ESSENTIAL PLAN EXPANSION INTO CLINTON COUNTY

In January 2025, Fidelis Care expanded its Essential Plan coverage into Clinton County, extending access to affordable, high-quality health insurance for more residents in Northern New York. Eligible residents now have access to benefits including dental and vision care, free preventive services, and a fitness reimbursement program.

This expansion reflects Fidelis Care’s mission to close coverage gaps and ensure that every New Yorker, regardless of income or location, can access the care they need to live healthier lives.
