SOUTH WOOD COUNTY POPULATION HEALTH STEERING COMMITTEE • The purpose of the Steering Committee is to organize key community stakeholders to develop a coordinated approach to prioritizing and addressing population health and provide guidance on the direct placement of UWSP resources within South Wood County area. • Current organizational members include: Wood County Health Department, Aspirus-Riverview Hospital, Marshfield Health Clinic, Port Edwards School District, School District of Nekoosa, Wisconsin Rapids Public Schools, United Way of South Wood and Adams Counties. • Drafted steering committee charter that clearly outlined member roles and responsibilities. • Members unanimously voted on setting first priority focus on youth health initiatives for strategic goals.

DATA USE AGREEMENTS

• Negotiated with the Wisconsin Hospital Association (WHA) to obtain data agreements for our work as a steering committee. • The South Wood County Population Health Steering Committee is the third group in the state that are using WHA data for purposes of improving population health. • The first group was the collaborative formed by the network of Milwaukee hospitals for their community health improvement, and the second group is in Dane County for similar work by the hospitals systems in Madison and the surrounding areas. • Provides the ability to create timely zip-code level measures to track and monitor over 80 health outcomes and risk behaviors. • Over 250 gigabytes of data with continuous quarterly file updates for all hospitalizations and emergency department visits by person-level residential zip codes.
