The curriculum vitae should include a description of teaching and other educational activities, including teaching of medical students, residents, fellows, and graduate students. A summary of courses taught and lectures given must be included to document teaching. The curriculum vitae should also include a table of pre-doctoral students, postdoctoral students, residents and fellows, and junior faculty mentored; their roles while being mentored; dates; candidate’s role as mentor (research advisor, career advisor, personal advisor, etc.); and current positions of current/former trainees. A summary of clinical activities should be included, if relevant, as well as a description of teaching and other educational activities, including teaching of medical students, residents, fellows, and graduate students. Lists of committees should include dates of service, the full name of the committees, the organization for which the committee works, and a brief description (1 or 2 sentences) of the duties of the committee. 4. Initial letter of appointment. 5. Personal statement from candidate which includes a description of the individual’s career goals (not to exceed two pages). 6. Copies of recent annual evaluations (this folder will be pre-populated with the most recent evaluations). 7. Letters of recommendation a. Reappointment: At least three letters of recommendation from faculty who are in a position to judge the quality of the candidate’s activities are required except for faculty members appointment for a one-year term. b. Promotion from Instructor to Assistant Professor: At least three letters of recommendation from faculty who are in a position to judge the quality of the candidate’s activities are required. c. Promotion from Assistant to Associate Professor, and Associate Professor to Professor as well as Tenure Review: • Clinical Investigator Track, Research Tenure Track, and Education Track At least three letters of recommendation from individuals outside the University of Cincinnati are required. It is recommended that these letters be from individuals in other scholarly institutions who are
