The student and older adult meet to discuss the person’s current healthcare status and needs, understand healthcare beliefs, cultural and spiritual needs, and their support system. After discussing the person’s needs to assist with their health and safety the student shares available resources they have identified. The student also assists the older adult to fill out the Oregon Health Authority Advance Directive Form. Three weeks later the student meets with the older adult again to discuss progress on sharing the advance directive and person’s wishes with their support person. They discuss the use of any identified resources and need for further support. During the final capstone practicum experience the assigned faculty member is responsible for evaluating the student’s performance and all associated assignments completed during the practicum experience. Throughout the practicum students will prepare a weekly journal discussing how they have met course outcomes or shown understanding or mastery, how they have demonstrated the new role as a baccalaureate generalist nurse and what changes they see in this new role from practicing at the associate degree level, reflect on how they have grown as a professional nurse, developed confidence or self-image in the new role, and describe what changes they are experiencing. During the final practicum the student completes the required hours of direct care experience. The students are also required to complete an assignment with a patient who is receiving regenerative or restorative care. While providing direct care to a patient/patients at the capstone site the student must assess ta patient’s knowledge of healthcare technology and ability to utilize any technology utilized in providing their care. This could be the use of technology to provide patient teaching, use of healthcare portals for the patient to receive and send medical information between the patient and provider. Bedside technology, wearable technology, or implantable technology are other examples. The student will assess the knowledge and ability of the patient to utilize the technology, plan to provide further education or assistance, and evaluate patient’s outcomes of the interventions.
