III-H 2. Describe how clinical practice experiences foster interprofessional collaborative practice:
Throughout the clinical experiences of the BSN program students work with a variety of healthcare team members. They spend time in the beginning of the program working beside the CNAs and LPNs to develop understanding and respect for their roles on the healthcare team. Students gain knowledge of and practice delegation. Students demonstrate hand off using SBAR at the end of shift sharing information with the oncoming nurse. Students learn and practice collaboration with other departments including the dietitian, physical therapy, occupational therapy, respiratory therapy, and social services to enhance patient outcomes and quality of care. During the practicum experience students in collaboration with their mentor will assess the needs of the facility, organization, interprofessional team, patients, or populations they are working with during the practicum as well as their own learning needs. Based on the assessment and input from the mentor the student will determine a change project that will have a positive impact on the healthcare team or patients at the practicum site and demonstrate mastery of course learning outcomes. The student will implement and evaluate the project. The project is presented to their peers in the final week of the practicum course.
