SUPPORT LIST RESPONSES V012059: Operators with losses in excess of retention, if the loss involves a late report to NIIC. V012059: We are working to address the deficiency noted at the most recent risk management visit. Recently, we have designated additional administration help to assist with the safety responsibilities to ensure sustained improvement can occur. To avoid delays in our loss reporting, below are changes that will be made in the near future: • Reporting Process Cheat Sheet and FAQs : We currently have a reporting process cheat sheet along with frequently asked questions {FAQs) in a place that is accessible to all employees. We will begin sending our reporting process documentation and FAQs out on a quarterly basis. We believe this will keep our reporting process at the forefront of our day-to-day operations and will help combat late reporting. • Retraining Sessions: We will conduct training sessions to familiarize employees with our current policy on reporting incidents and injuries. Our training will ensure that employees understand what to do in case of an incident or injury, give reasons on the current structure of our incident handling and encourage employees to use the provided tools, training and guides made available by our company.

We are working to address the deficiencies causing our organization to be placed on the support list. Specifically, we had a claim, that was a loss in excess of retention and was reported late to NATL. The late report was due to communication error between departments. Below, we have listed the organizational changes that have been made when it comes to the reporting of worker's compensation claims. These steps should prevent further issues: •All reports of injuries, regardless of severity, will be immediately reported to the employee's supervisor. •If the employee's supervisor is out of the office that day, the employee will report the injury in email format with all thr ee supervisors copied on the email. •Reporting will be made regardless of whether the employee believes the truthfulness and veracity of the person reporting the injury. •We have designated one supervisor to manage claim reporting to National Interstate. If that supervisor is unavailable, we have designated a list of supporting supervisors to report claims. •These policy changes have been reported to all personnel in both written and oral format.
