When you return from donating blood, you may be asked to provide a written statement from the blood bank confirming you kept your appointment. You will retain the hour of paid leave if your attempt to donate blood is unsuccessful. However, you will be required to provide the Company with a written statement from the blood bank confirming you kept your appointment. You may request blood donation leave for an unsuccessful attempt no more than once per 56- day period, unless otherwise approved by the Company. The Company will not retaliate against employees who request or take leave in accordance with this policy. Leave for Victims of Domestic, Sexual, Gender, or Criminal Violence (VESSA Leave) In accordance with the Illinois Victims' Economic Security and Safety Act (VESSA), CREA, LLC will provide unpaid leave (VESSA leave) to eligible employees who: Are victims of domestic violence, sexual violence, gender violence, or any other crime of violence; or Have a family or household member who is a victim of domestic violence, sexual violence, gender violence, or any other crime of violence.
Qualifying Reasons for Leave VESSA leave may be used to:
1. Seek medical attention for, or recover from, physical or psychological injuries caused by domestic violence, sexual violence, gender violence, or any other crime of violence against you or your family or household member. 2. Obtain victim services for you or your family or household member. 3. Obtain psychological or other counseling for you or your family or household member. 4. Participate in safety planning, including temporary or permanent relocation or other actions, to increase your own or your household or family member's safety from future domestic violence, sexual violence, gender violence, or any other crime of violence. 5. Seek legal assistance to ensure your own health and safety or that of your household or family member, including participating in court proceedings related to the violence. 6. Attend the funeral or alternative to a funeral or wake of a family or household member who was killed in a crime of violence. 7. Make arrangements necessitated by the death of a family or household member who was killed in a crime of violence. 8. Grieve the death of a family or household member who was killed in a crime of violence. Duration of Leave Except as otherwise provided below, you may take up to 12 weeks of unpaid VESSA leave within any 12- month period. Leave is based on a rolling 12-month period, looking back from the date the leave would begin. Leave under this policy may be taken intermittently or on a reduced schedule basis. You may not take more than two workweeks (10 workdays) of leave for the purposes described in bullets 6, 7, and 8. Leave under these circumstances must be completed within 60 days after the date on which you receive notice of the victim's death. Notice and Certification To obtain leave under this policy, you must provide the Company with at least 48 hours' notice, except in emergency situations or where such notice is not otherwise practical. In all cases, either before or after you take leave under this policy, the Company will require you to submit a sworn certification that your absence is for one of the qualifying reasons listed above and that you or a family or household member is a victim of domestic or sexual violence.
