Executive Summary Trinity Safety Group (Trinity) is proud to present FCA with a proposal for EHS Construction Safety Services. This proposal describes Trinity Safety Group’s professional services and fees associated with the development and facilitation of an approach to pro-actively support and provide assistance to FCA in order to improve safety, reduce losses and achieve specific financial results. Trinity understands that safety is imperative to a successful business and promotes safe production through a unified commitment from all parties involved on the project. Based on a successful track record of project safety management, improved safety behavior, and reduced incidents in a variety of industries, Trinity is uniquely structured to guide FCA in instilling safe production as the priority on every project. Trinity’s services include: design of project-specific safety and risk management programs in collaboration with FCA management, contractors, and EHS staff; and implementation and oversight of a robust on-site safety management program. Our Company For more than 15 years, Trinity Safety Group has provided on-site safety management services to clients across the nation. Trinity performs work in a variety of industries including construction, manufacturing, automotive, healthcare, pharmaceuticals, oil and gas, chemical, logistics, industrial, and mining. We have implemented extensive safety programs on projects including power generation and automotive plants, refineries, MSHA, high-rise hotels, hospitals, stadiums, multi-story office buildings, and more. Trinity’s team consists of top-level experts in the safety industry with extensive operations-specific knowledge. In addition, Trinity has a staff of talent recruiters, a national database that enables us to find top safety professionals, and a network of organizations across the country. Geographic Reach, Proven Process and Leadership Depth To create efficiency, fast site safety staffing response times, and a deep bench, Trinity has partnered with Core Safety Group. Trinity Safety Group will remain as the lead safety provider, but leverage Core Safety’s geographic footprint. Together Trinity and Core Safety will have five locations:

Two companies; one philosophy to generate a cohesive team to support your safety efforts. Together Trinity and Core employ more than 85 safety consultants of which two are bilingual Spanish speaking safety professionals. We understand that safety can be challenging at times and do not want language to increase the risk of an accident. To this point, we will use our Spanish speaking safety professionals and others on the projects that requires this skillset to deliver safety oversight.

Trinity Safety Group and Core Safety have experience working outside of the United States. Trinity has delivered high level safety culture and behavior-based safety guidance for clients in the Middle East and Core safety has provided site safety staffing to clients in Canada. Our recruiting department has the capability for sourcing individuals to work all over the world. We understand it can be challenging working outside the US. As needed, we have hired consultants such as attorneys and accountants to support us in these endeavors.

A Turn-Key Approach-More than just a Professional Trinity Safety Group has represented large owners with multi-site construction projects across the United States. We have a successful track record of project safety management, improving safety behavior and reducing incidents and injuries during construction and start-up. Trinity is uniquely placed to assist in fulfilling the vision of a worksite with safety and production as the priority. We are confident that our proposed services will effectively address your needs. Trinity Safety Group offers a turn-key solution for site safety management. Our engagement with the project starts with team selection. We work with our clients to select a safety professionals who fit the organization’s personality and goals. Utilizing an in house recruiting department, Trinity Safety Group has built a network of safety professionals who are pre-screened and ready to provide meaningful, result oriented solutions for your projects. We select only highly qualified safety professionals with both practical “boots on the floor” experience and education. Our safety professionals take ownership of the program and provide guidance and solutions. Trinity Safety Group believes there is a process to ramping up for site safety management effectiveness. We try to do more than just provide a person for your project. In addition to providing the professional, we will make sure they are trained and understand your safety system. We want our safety professionals to become a part of your team, deliver results and exceed your expectations. To do this we use the follow technique and approach: Exploratory The purpose of the Exploratory phase is to establish a high level of competency regarding current processes, procedures and incident history. Anticipated activities in this phase consist of the following: • Review of existing policy, procedure and cultural initiatives to establish consistencies and best practices of each. • FCA and industry-related incident data interrogation regarding events of activities similar to those detailed in the operational scope of work for this project. • Establish communication, coordination and permanent presence with FCA and contractors. Implementation Building relationships and holistic project safety management integration of processes, between FCA, and Trinity anticipates this phase to include heavy involvement and interaction, all parties to establish alignment of project vision, objectives and activities. The conclusion of the Implementation phase will have all parties effectively prepared and aligned for a unified approach resulting in safe production on the project(s). Deliverables may include: • Project activity pre-planning – Hazard Identification and Risk Assessments. • Contractor on-boarding preparation. • Preparatory planning meetings. • Establishing communication channel and methods.

On-site Safety Delivery A full-time Safety Manager will be on site with the start of construction activities. The Trinity safety professional will have all the tools needed to facilitate site safety for FCA. Attention will be paid to monitoring and measuring performance with the purpose of identifying strengths and improvement opportunities. FCA, contractors, and Trinity will work together to recognize high levels of performance and when necessary, develop improvement initiatives. Relentless effort will be demonstrated by personnel to strive for continuous improvement, through extensive extraction and use of data from observations and event investigations. Deliverables: • Coordination, administration and implementation of FCA Safety programs. • Review and provision of feedback for Job Safety Analyses submitted by the Prime Contractors. • Assistance with the development of contract safety specifications. • Development and implementation of accident prevention plans, to include on-site inspections and review of contractor performance. • Investigation and coordination of all accident reporting, and facilitation of the investigation process. • Provision of monthly reports of each contractor’s safety performance. • Attending all pre-bid and pre-construction meetings to ensure contractors understand the OCIP process and policies. • Pre-planning activities with the contractor. • Conduct an effective site safety orientation. • Conduct daily project safety observations and reports of safe/unsafe acts and conditions. • Review of site-specific safety plans. • Review the contractors’ training and maintain a tracking system. • Review of emergency procedures to include rescue plans and communication processes. • Providing accountability and leadership to the contractor. • Conduct weekly safety meetings with key stakeholders. • Facilitate a project safety committee consisting of management and craft personnel. Case Study Overview of Engagement

Trinity Safety Group delivers on-site safety services for construction related work on a design build metro rail project and two Rolling OCIPs, that include above ground and tunnel construction related to rail operations and another to water and sewer infrastructure. Client Profile: Large Metro Area Transit Rail Operation Construction value: $2.7 B over five years Location: Mid-Atlantic Description of Trinity Services and performance: Trinity Safety performs in a full-time loss control/safety capacity on-site for the metro rail OCIP reporting to a Wells Fargo Insurance Safety professional. The purpose of the position is to observe daily activities at the job sites and make sure safety protocols are being followed. The Trinity professional receives a weekly list of contractors and has access to our web-based Portal which provides up-to-date information on whether or not contractors have met the requirements for enrollment in the OCIP. The current professional assigned to this contract has been an asset to the engagement through his professional manner, team spirit, and attention to detail. Client Profile: Metro area transportation and light-rail operation Construction value: $750 M over five years Location: Mid-Atlantic Description of Trinity Services and performance: The initial engagement for Trinity Safety mirrored that of the above described engagement. The exception being that this client has an in-house safety team that Trinity supports. The performance of Wells Fargo Insurance Safety team provided exceptional value to the client and in Q2 2016, the client contracted with us on an expanded basis to replace their in-house construction safety professionals. Trinity Safety participated in the interviews to replace the team, spearheaded the transition and seamlessly continued their work on a more expanded basis. The politics of the transition process was a challenge for Wells Fargo Insurance and for Trinity. Trinity Safety leadership was particularly responsive and flexible during this time. In addition, the safety professionals assigned have performed well. The client is considering additional expansion of this contract to include another large project. Client Profile: Large Water & Sewer Operation Construction value:$ 1.2 B over five years Location: Mid-Atlantic Description of Trinity Services and performance: Wells Fargo Insurance contracted with Trinity Safety to provide a team of safety professionals to monitor over $1.2B in construction value over a 5 year period. Responsibilities are similar to those described previously. The exception, this engagement required a Safety Director be hired immediately and that person worked with Wells Fargo Insurance and the client to build a team around the specific needs of the project. The client changed brokerage firms and contractors were still working with a previous safety team, leading to confusion. The education and coordination that needed to take place was a challenge but Trinity Safety has performed well on this contract. We are well into year two and Trinity Safety professionals continue to contribute in terms of job performance and collaborative spirit; making recommendations that have proven valuable to the project and the client.

Jason Jones – President As President of Trinity Safety Group, Jason leads and directs the company in support of its goal of exceeding expectations and delivering results for safety support and services. Jason’s nineteen-year career in the field of safety has included immersion in risk management services, construction project safety management, and implementation of strategic safety culture initiatives for both US and global corporations. Jason founded Trinity Safety Group in 2002 on the core principles of developing processes that are meaningful including keeping safety about the people. Jason and his team have built a systematic approach for safety management that facilitates extraordinary levels of performance. With extensive hands-on experience in safety systems and project delivery, Jason is responsible for leading Trinity Safety Group in developing, managing, and conducting

complex safety and risk management solutions for numerous clients in several industries. Jason is a recognized expert in large project risk management and has been involved with hundreds of projects across the country, totaling over $25 billion in value. Jason and his team additionally are lauded for their expertise in OCIP/CCIP project delivery. Prior to founding Trinity, Jason was a Corporate Safety Manager for a national construction management firm where he led the safety department team, developed and implemented cultural initiatives, and provided risk management oversight of all business units. Jason has also fulfilled the role of Project Safety Manager for many construction projects in the Midwest where he led behavioral based safety, executive leadership coaching and safety culture initiatives for large-scale construction projects throughout the United States. Jason holds a Bachelor’s degree in Safety Management and is a member of the American Society of Safety Engineers (ASSE). Jason also lends his experience and talents to the Coalition for Construction Safety serving as a member of the Safety Director Council. He is also the Committee Chair for Youth in Construction within the Indiana Contractor Association.

Russ Bartholomew – President Russ is the President of CORE Safety Group. Based out of the Chattanooga Tennessee office, Russ is a multi-degreed safety professional with over 15 years of experience in the development of corporate safety programs for both general contractors and specialty contractors. He has hands-on experience with developing jobsite specific safety plans and job site hazard analysis for critical construction activities in aviation, major sports facilities, large industrial, chemical

and power generation. He has developed extensive safety training programs for supervisors and continues to train workers in all aspects of construction safety. Russ has successfully administered several owner controlled insurance programs (OCIP) and is experienced in the development of loss control programs to aggressively manage claims. Russ has a Bachelor of Science degree in Occupational Health and Safety and a Bachelor of Science degree in Construction Management from Indiana State University. Russ holds the Certified Safety Professional (CSP) and Construction Health and Safety Technician (CHST) certifications from the Board of Certified Safety Professionals (BCSP). He continues to be a leader in the safety community and participates in many local and national safety committees including the AGC of America and American Society of Engineers. Kraig Kyle – Vice President Based out of Dallas, Texas, Kraig oversees company

operations and services. Kraig has over 13 years of practical safety management experience that includes developing and managing corporate safety programs and advising client leadership on best management and loss control practices. He has hands-on experience with developing jobsite specific safety plans and job hazard analysis for critical activities in industries such as pharmaceutical, healthcare, commercial, and power generation. Kraig has experience in loss control such as workers compensation, general liability, and claims management. These experiences include effectively developing and implementing safety and claims management strategies on billions of dollars of wrap- up insurance programs. Kraig is a graduate of Indiana State University with a

Bachelor of Science degree in Occupational Health and Safety Management and currently holds the Certified Safety Professional (CSP) and Construction Health & Safety Technician (CHST) certifications from the Board of Certified Safety Professionals.

Proposed Safety Team Trinity Safety Group’s proposed team consists of experts in the safety industry with over 75 years of combined experience. Trinity and CORE’s teams provide a diverse group with knowledge of cultural, behavioral, and traditional safety management.
