INITIAL APPOINTMENT

In the cases where the College ARPT must consider the initial appointment of a new faculty candidate, an original and nine copies of Documentation Materials in ten separate files must be submitted by the faculty member to the Department Chair’s office for consideration by the appropriate ARPT Committee. In other cases, where only the Dean must consider the initial faculty appointment, an original and one copy of Documentation Materials in two separate files must be submitted to the Department Chair’s office.

The materials to be submitted by the faculty for initial appointment include:

1) Record of Action form. 2) Department Director’s recommendation in letter form which includes a proposed job description with a breakdown of time and effort in percentages, and approval signature on Record of Action form. 3) Departmental ARPT Committee Chair’s approval signature on Record of Action form (not required for Volunteer and Visiting Tracks). 4) Current curriculum vitae of faculty candidate. The required format can be accessed on the DFCM intranet https://fammed2.uc.edu/default.aspx 5) Letters of recommendation – at least three (3) letters of recommendation from faculty who are in a position to judge the quality of the candidate’s activities are required. These letters are in addition to the Department Director’s letter. 6) Personal statement from candidate which includes a description of the individual’s career goals (not to exceed two pages). 7) Teaching Evaluations (if applicable

REAPPOINTMENT

The following materials must be included in dossiers submitted by current department faculty in February for consideration by the ARPT Committee for reappointment.

1) Record of Action Form (Title Approval Request Form for Volunteers, Secondary or Adjunct Tracks) 2) Department Director’s recommendation in letter form, and approval signature on Record of Action form. 3) Departmental ARPT Committee Chair’s approval signature on Record of Action form (not required for Volunteer and Visiting Tracks) 4) Current curriculum vitae of faculty candidate. The required format can be accessed on the DFCM intranet https://fammed2.uc.edu/default.aspx 5) Letters of recommendation – at least three (3) letters of recommendation from individuals who are in a position to judge the quality of the candidate’s activities are required except faculty members appointed for a one-year term. One of the three letters should be submitted by the candidate’s immediate supervisor.
