FAMILY HEALTH REGISTERED NURSE Temporary, Full-time Position (One year maternity leave replacement) (37.5 hours per week) The Lower Outaouais Family Health Team is recruiting for an experienced Registered Nurse that will work collaboratively with a variety of healthcare professionals in a primary care setting. Reporting to the Executive Director, in collaboration with the Lead Physician, you will assess, plan, implement and evaluate nursing care for FHT patients, as part of an inter-professional team. You may have a wide range of responsibilities including, but not limited to, well-baby care, patient navigation, chronic disease management program development, planning, implementation and evaluation. This position will require using your full scope of nursing practice. As a qualified Registered Nurse with current CNO registration, RNAOmembership and basic CPR certification, you bring knowledge of current, evidenced-based methods and practices of primary care delivery.
