could lead to more long-term issues with back pain — not fewer. Exercise, in general, is shown to increase strength and flexibility, supporting healthy muscles and bones, and therefore supporting ideal back health. Physiotherapy for Back Pain While rest and relaxation can help you overcome the immediate pain of a back injury, and may even be recommended by your physician in the early days following an injury, it is not a long-term solution. Physiotherapy offers a long-term solution to back pain by using targeted exercises that focus on the cause of the pain. Through a combination of strength and flexibility training that focuses on muscle development and joint movement, physiotherapy can address the underlying cause of the pain and significantly improve your quality of life.

In ages past, back pain was difficult to treat. If you experienced back pain, whether as a result of a work injury, trip-and-fall accident, or even just as a result of aging, the answer was almost always the same: head home, take a long rest, and give your back time to heal. This isn’t the way that things go anymore, and forseveral reasons.Tostart, theworld isn’t as forgiving. Heading home and taking a long rest until your back is healed may work for some, but not for most. With deadlines and carpools and work schedules to keep up with, there needs to be an alternate solution to dealing with back pain that doesn’t require you to completely remove yourself from your responsibilities. What’s more, recent research indicates that resting may not actually be the ideal solution for long-term back care. Spending too much time on the couch or off your feet can cause the back muscles to weaken and can even weaken bone strength. This

UNDERSTANDING WHY YOUR BACK HURTS

There are a lot of different reasons that back pain can develop. Even when you break down injuries, whether from overuse or athletic pursuits, there are different problems that can develop. Sprains and strains are common, but so are issues with the vertebrae, blood flow, and even concerns regarding the spinal nerves. You might be amazed to discover the different factors that could be influencing your back health. Such as: • Your personal level of physical activity, including how often you exercise and the intensity of your typical workouts. • The types of shoes that you wear, in addition to how frequently you walk in different types of shoes, particularly shoes that lack support or those with high heels. • Prolonged engagement in sedentary behavior, including sitting at a desk for eight hours or more consecutive days of the week, or spending free time on the couch, or otherwise relaxed. Aerobic activity and strength training exercises actually make it possible to reduce your risk of injury and to improve your ability to overcome back pain by strengthening the vertebrae and improving blood flow and nutrient disbursement throughout the back. When you are inactive, blood flow can actually become impeded, and this can have a negative effect on the overall health of your back and spine.

How Physiotherapy Helps Anyone who has struggled with back pain can tell you plain and simple: When your back is hurting, there is no way to pretend that it isn’t. Simply going from sedentary activity to being active and healthy isn’t an option — at least not so easily. It takes time and effort, and when back pain is obstructing you from getting started, it requires help. Physiotherapy can help you overcome back pain by giving you the knowledge and support necessary to help your back feel better, giving you the option to get off the couch and push yourself to reach new goals. Working with a licensed and experienced physiotherapist ensures that you do not take on too much too quickly, but instead are guided through the process of healing with gradual steps. DRY NEEDLING BY SANDEEP KUMAR RAJAVELU BALACHANDER

Dry needling is a specific clinical technique used by soft tissue therapists to control pain, reduce muscle tension, normalize biomechanical and electrical dysfunction of motor endplates, and facilitate an accelerated return to active rehabilitation (AAOMPT 2009). It is a technique that is used to treat dysfunctions in skeletal muscle, fascia, and connective tissue to help diminish persistent peripheral nociceptive input that enables reduction of impairments of body structure and function, which eventually leads to improved activity and participation in activities of daily living. Dry needles are solid filiform surgical steel grade needles that are used to penetrate the skin and stimulate underlying myofascial trigger points. This contrasts with the use of hollow hypodermic needles to inject substances like saline solution, Botox, or corticosteroids to help relieve pain and discomfort. Pain and dysfunction in soft tissue is the result of taught bands or inflammatory muscle fibers which develop trigger and/or tender points as well as painful areas along the flow of the peripheral nerves or the pain pathways. In lay language, instead of digging our fingers into the sore parts, we can insert a needle to do the same job.

Needle insertion can be done at areas of myofascial tension (commonly referred to by patients as ‘knots’), bone periosteum (where tissues fixate to bone and are the source of pain i.e. shin splints) or other soft tissues. As the needles are inserted into the areas of tension, the focal invasion causes the tissue to react. This reaction is promptly followed by a degree of tissue relaxation and hopefully a reduction in pain. Dry needling is helpful when treating a large area of muscle tension. In addition, patients who are sensitive to touch when in acute pain often find dry needling extremely effective as their tightness can be released with minimal hands-on therapy. Dry needling is indicated in the following conditions but not limited to them: Tennis Elbow, Golfers Elbow, Sciatica, Slipped Disc, Neuropathy, Osteoarthritis of the knee joints, Carpal Tunnel Syndrome, Frozen Shoulder, Calcaneum Spur, Plantar Fasciitis, Cervical Spondylosis and radiculopathies, Chronic Migraine, Temporo Mandibular pain and stiffness, and excessive leg swelling. The healing pattern is not predictable and varies from person to person depending on the condition and the nature of pain. The results are quicker in acute conditions as opposed to chronic conditions. In general, it’s safe to say that it takes a few days to see results and they are cumulative too.

OCTOBER IS NATIONAL PHYSIOTHERAPY MONTH!

What Is National Physiotherapy Month All About? Did you know that the entire month of October has been dedicated to National Physiotherapy Month? For almost 40 years, physiotherapy has been celebrated by official proclamation. And since 1992, October is the month for honoring physiotherapists and the important role they play in healing people in pain. In the modern era of social media, hashtags like #UsePhysicalTherapy and #ChoosePT have helped spread the word. Physiotherapy Month allows patients to share their stories about how PT has changed their lives for the better. It also gives physiotherapists a forum for spreading the word about the different kinds of treatment patients can receive. Many members of the general public don’t realize that aside from specialized exercises that build strength and flexibility, PT also offers pain management through massage, heat and cold applications, and specialized electronic-pulse devices. What Symptoms and Conditions Does PT Help? Physiotherapy tends to be associated with sports injuries, along with chronic problems such as back pain, surgery recovery, and balance issues. Yet, you may not realize the range of problems which physiotherapy can help -- both for pain management and to help ease other symptoms. Do you have hand pain and stiffness? Physiotherapists treat hand and wrist conditions ranging from carpal tunnel to “trigger finger.” Tennis elbow and rotator cuff injuries can also be aided through PT. Among other conditions the general public may not associate with physiotherapy are pelvic floor dysfunction, TMJ, cardiopulmonary disease, Parkinson’s, and traumatic brain injuries. Why #ChoosePT? In recent years, National Physiotherapy Month has also focused on the alternative PToffersoveropioids.Withopioidabuseatanall-timehigh --and tragicoverdoses on the rise -- alternatives to this class of prescription medication are even more important. The National Centers for Disease Control (CDC) estimates that opioid prescriptionshavequadrupled in recentyears.Yet, thenumberofpeople reporting chronic pain has not gone down. Sohowcanyoutell ifphysiotherapy isabetterchoicethanprescriptionopioids? The CDC suggests that if you fall into any of these categories, physiotherapy can be beneficial: •Pain comes from fibromyalgia, low back pain, knee arthritis or hip arthritis. These conditions in particular are known for responding well to physiotherapy when it comes to pain management.

•The pain has lasted more than 90 days, meaning that it has gone from “acute” to “chronic.” •The benefits of prescription opioids don’t outweigh their risks. In the early days after an injury or an operation, painkillers help patients function without crippling pain. But with each day that passes, the risks opioids pose outweigh the rewards. If you’ve become afraid of the pain. Obviously, nobody welcomes physical discomfort. But some people become overly dependent on a pill’s ability to remove all aches and twinges. In PT, pain is a way to determine exactly what needs to be worked on, in order to reduce discomfort down the road.
