We want to notify you of a computer backup hardware failure which occurred in August of 2017. This resulted in a loss of medical records between July and August. Rest assured, the data was not stolen, only erased. The hardware had a malfunction and the data files could not be retrieved despite numerous attempts by teams of computer technicians and engineers. Since this occurrence, we have redoubled our efforts to ensure this rare event does not happen in the future. We sincerely apologize if you have been inconvenienced in any way.
