For a while, Fyzical El Paso didn’t have a program of this sort. Whenever we take on something new, we want to make sure we have all the resources we need tomake it as successful andhigh-quality for our patients as possible – and for a while, we didn’t. At Fyzical El Paso, we like to take our time to ensure we are doing everything right. It can sometimes be a very long process! It all started a few years back when a patient came to us seeking help for pelvic health. She had been having intimacy troubles and even flew all the way out to Philadelphia to get the help she needed. Consequently, she met with a physical therapist in Philly who I knew from my PT training, years back. He referred her to me, and while I didn’t have much experiencewithpelvic health, I wanted to try and help her. I consultedwith a nurse about the best ways to help provide this patient treatment and realized I already somewhat knew what to do. We provided the necessary treatments for her and they worked very well! Ever since then, I knew I wanted to make pelvic health an important part of our practice. Katy came on last year, expressing

At Fyzical El Paso, we like to hold each other accountable. I had stated in a previous newsletter that my resolution for the new year is to read more. Well, I am currently on my fourth book of the year and I wanted to share a little with you. I am currently reading The Price We Pay by Marty Makary. This book is a factual display of the public healthcare system and the abuses of it. It is uncomfortable but important, especially in times such as these, to understand the way healthcare works. If you are interested in giving this book a read, I highly recommend it – we will discuss it further in a future newsletter. UNDERSTANDING INCONTINENCE INSIDE: • Causes Of Incontinence &

We can also teach you habits and techniques to reduce urgency and urge incontinence (or leaking with a strong urge on the way to the bathroom). Even periodic incontinence is not something you have to deal with. We can help! Incontinence, Chronic Pelvic Pain, & Physical Therapy. A physical therapist is highly skilled in evaluating and treating variousmusculoskeletal and neurological conditions affecting the body. This profession focuses on assessing any underlying factors, such as strength, neuromuscular control, balance, coordination, skeletal alignment, and endurance, which may be contributing to impaired functional mobility, activity participation, and quality of life. Of course, most people only seek physical therapy when they’re suffering from a common injury or condition such as whiplash, tendonitis, low back pain, ligament sprains, and plantar fasciitis. Perhaps lesser known is that chronic pelvic pain and dysfunction, including incontinence, can also respond positively to physical therapy intervention.
Urinary incontinence has a reputation of being something only older people experience. But many young people get urinary incontinence. And while more women thanmenareaffected,men canhaveurinaryproblems too, especially after prostate surgery. Fortunately, there aremany treatments for urinary incontinence. Behavioral treatment. Some people with urinary incontinencemay get relief by making simple lifestyle changes. If you have stress incontinence, for instance, in which you leak urine when you cough, sneeze, or laugh, your physical therapist may tell you to limit howmuch you drink. If you have urge incontinence, inwhich you get the sudden urge to urinate and can’t always make it to the bathroom in time, your physical therapist may tell you to avoid spicy foods, caffeine, and carbonated drinks, because they can irritate the bladder andmake the problem worse. Exercises to strengthen the pelvic floor muscles, known as Kegels, can sometimes help people with stress incontinence. Kegels can also help people with urge incontinence. Sometimes, Kegels are combined with biofeedback techniques to help you know if you are doing the exercises properly. For urge incontinence, bladder training, sometimes called bladder retraining, can also help. This involves gradually increasing the interval time between trips to the bathroom, working up to longer and longer intervals between bathroomstops. Devices and absorbent products. Protective pads and panty liners can help avoid embarrassing situations. A pessary, a plastic device inserted into the vagina, may help prevent urine leakage by supporting the neck of the bladder; it is most useful for stress incontinence.

CAUSES OF INCONTINENCE & CHRONIC PELVIC PAIN
If you have pain in your pelvis, it may not always be fully clear what’s causing it. Some common causes of pelvic floor dysfunction and chronic pelvic pain include, but are not limited to: • Pregnancy • Sacroiliac joint dysfunction • Coccydynia • Irritation of the pudendal nerve (which originates near the bottom of your spine) • Irritable bowel syndrome • Levator ani syndrome (increased tension in pelvic floor muscles) • Post-surgical complications (e.g., pain following a hysterectomy, hernia repair, cesarean section, and/or prostatectomy) Any of these conditions may lead to inadequate support of the organs contained with your pelvic cavity, increased pressure or inflammation of nerves, muscles, and other connective tissues (including your bladder), and impaired pelvic bone alignment. These symptoms can be frustrating, painful, and embarrassing, and may limit your tolerance to daily participation in desired activities, including exercise. Other symptoms frequently appear with pelvic pain, including incontinence, bloating, abdominal pressure, reluctance to strain while defecating, and pain in the low back, buttocks, and hips. You may also have decreased tolerance to a variety of activities including sitting or standing, walking, exercise, and sexual intercourse. How A Physical Therapist Can Treat Pelvic Pain. Every patient presenting with chronic pelvic pain is unique. If you come to see a physical therapist for your pelvic dysfunction, you’ll be taken through a thorough physical examination and patient history questionnaire. He or she will ask you specific questions about your pain, what makes it worse and better, and whether you have a history of any related issues. Based on the results of the examination, your physical therapist can then develop a customized
treatment plan to relieve your symptoms and address the suspected underlying causes to prevent recurring dysfunction. Common physical therapy treatments may include: • Soft tissue mobilization and massage of internal and external pelvic musculature, to relieve muscle tension and provide pain relief • Spinal and sacroiliac joint manipulations to reset the neurological pathway, relieve pain, and restore normal spinal alignment. • Electrical stimulation modalities, such as neuromuscular electrical stimulation to provide pain relief and restore normal neuromuscular activation of pelvic floor muscles and nerves. • Therapeutic exercises to restore the normal strength, flexibility, and endurance of stabilizing postural muscles in the low back and hips. As a strength and conditioning coach, what attracted you to coach young athletes and students? Every athlete is born with a unique talent and has the potential to achieve success. I wanted to harness that talent and unlock every athlete’s potential to become the best they could be. Having worked at two different universities and trained hundreds of athletes from different sports, I wanted the opportunity to provide that same level of training to younger athletes. Quote: “Failure is a key to success. Failure is not the opposite of success, but a steppingstone to success.” What do you find most rewarding about being a strength and conditioning coach? Watching athletes grow every day, helping them reach their goals, and having a positive impact on their life, on and off the field/court, is its own reward. Who (or what) inspired you to become a teacher/coach? My father who was a basketball coach inspired me to become a coach. It was instilled into me at an early age, that to be a successful coach, you must be passionate about listening, caring and working with athletes.

In last month’s newsletter I had information about pain and I had lots of responses so I thought I’d offer some additional information and maybe some help. Do you have pelvic pain when you sit? Roll over in bed? Get in or out of the car? Drive long periods? If you have pain with these normal daily activities… there is a simple exercise you can do for relief… and get back to feeling normal again….
Hi Fyzical member, last month I gave you lots of information about pelvic pain including some common causes. If you missed it, pop in and we have a few copies left over.
It’s frequently not picked up on MRI or X-ray. 3 Common causes are: 1. Herniated or slipped disc 2. Stenosis, arthritis, and disc degeneration
3. SI joint and Pelvic pain Now the 3rd cause… SI joint and Pelvic Pain, is a super tricky one.
And we’ve seen a lot of people suffering with pelvic pain and incontinence who’ve tried everything else (including chiropractic, medications, hot packs, inversion tables, injections, smelly blue lotions, failed surgery… and even other PT)… Yet still suffering.
Many times, these people have hidden a cause of their pain… a problem in the SI Joint or Pelvis.
So the person might have had treatment for a disc problem or arthritis… but they never had treatment to address the real cause of their problem… the SI joint. SI joint sufferers usually have pain with changing positions such as getting in or out of a car…or rolling over in bed… or getting in or out of bed. Sitting or driving is usually a problem…and one leg can feel “heavier” than the other.
Sufferers with this cause can even experience groin pain and trouble controlling their bladder (even at a young age).
Most people we see with SI joint and pelvic problems will have pain on either side of the tail bone or in their groin.
When you know the cause of your pelvic pain and incontinence, then it’s much easier to do the right treatment to get back to normal….. and heal naturally. Meet Our Pelvic Floor Specialist Kate Roby, PT, DPT
Kate is one of our valued team members who puts her all into treating patients. She initially started college as a pre-pharmacy major but later changed her focus to physical therapy after learning about the profession. Kate says, “I liked the idea that the body can heal itself naturally in many cases, if just given the correct tools.” She utilizes this principle while creating customized programs for her patients based on their situations.

Since starting her career in physical therapy, Kate has committed to continuing her education by specializing in pelvic floor health and rehab, after learning how common these issues are and how much they can adversely impact someone’s life. Kate wants her patients to know that you are never alone if you are experiencing issues such as pelvic pain and incontinence.
When asked why she is passionate about pelvic health, Kate said she believes that, “often people are afraid of confronting and discussing these issues. It can quickly feel like you are the only one dealing with them.” However, she knows that no patient is in their battles alone, and she aims to help them discover that. Kate also sees many patients who are frustrated after receiving previous treatments for their issue(s). For this, she provides some words of wisdom: “It can be frustrating after you feel like you have tried every treatment, but don’t give up!”
At the end of the day, Kate wants people to know that there is help out there and that it does not have to be an overwhelming or intimidating experience when seeking help for a certain condition. In fact, she wants patients to know that, “part of what we as PTs do is finding and working within your comfort zone, providing education so that you understand your treatment.” Kate believes the key to rehabilitation of any injury or condition is considering the overall dynamic wellness of the patient as a person. She has a passion for physical therapy because it’s “a unique opportunity to really spend time getting to know our patients and seeing them progress throughout the treatment process.” Outside of the clinic, Kate enjoys spending time with her family, traveling, hiking, watching documentaries, and sketching. She stays true to her philosophy that “life is about balance!”
