Additional occupant (based on Administrative approval) will be ½ the daily rate/day. Daily Rate Includes : room, housekeeping services, social services, linen service/Laundry, meals (including special diets), *activities, standard cable service, and local telephone service. Pets : $150.00 non-refundable up front per animal plus $10.00 a month per animal *Occasionally, activities that are planned outside of the facility may be charged in addition to the published room rate. Not Included in Daily Fee 1. Oxygen Therapy: concentrator - $1.25/hour; high volume concentrator $3.00/day; Portable tank - $9.00/tank 2. Isolation/Precaution Supplies: $15/day 3. Beauty/Barber: Haircut Men $17.00 Haircut Women $19.00 Shampoo & Set $24.00 Shampoo, set & cut $35.00 Permanent w/Shampoo, Cut & Set $70.00 Highlight, Shampoo, & Set $70.00 (Village Apartment Salon) Hair color, Shampoo, & Set $57.00 Manicures $15.00 Pedicures $50.00 (Village Apartment Salon) Wax $ 9.00 Wax Lip,Chin, & Eyebrow $13.00 4. Medical, dental, and eye appointments requiring St. Anne ’ s transport services will be billed based on the destination zone and the type of transport provided. Please see reverse side for rates. 5. There will be a $35/hour charge if an employee is needed to be in attendance during medical appointments. If a family member wishes to accompany a resident to a medical appointment, please contact the Transport Office at 285-6125. 6. Use of van for non St. Anne ’ s Retirement Community sponsored activities will be determined at time of request. Other Items Not Included in the Daily Rate 1.
