• To create your account to access the at-home care plan, you will need to log in using the email you provided in your New Patient Paperwork. • Enter your Name, Date of Birth, and Personal Access Code to gain access! NOTE: Your Access Code was provided on your printout at your first visit, it was also e-mailed to you. If you can’t seem to find it, feel free to give us a call! 2. Start your plan!: Access your health care plan including at-home exercises, track your progress, directly message your primary doctor/therapist, and reach your recovery goals!

EXERCISES TO COPE WITH STRESS NECK NOD Start with your neck in a neutral position, then slowly lower your head backwards and then forwards. Repeat 10 times. SINGLE SHOULDER RAISE

Exercise in almost any form can act as a stress reliever. Being active can boost your feel-good endorphins and distract you from daily worries. Try one or both of the exercises below! Discover how activity and mobility can combat any stress you may be experiencing.

Start by standing with good posture and your arms by your side. Raise one shoulder to a 90 degree angle and hold for 10 seconds. Repeat 5 times on each arm.
