This questionnaire will be sent to your email via our Jane booking system 24 hours before your appointment. You will not be allowed to enter the clinic prior to having our form filled out and signed. Arriving at the Clinic To reduce the amount of people at the clinic during the same time, we ask that you arrive at the clinic exactly at the time of your appointment. If you are early, we ask that you please wait outside or in your car. Before Entering the Clinic Please bring a mask if possible and put your mask on. If you do not have a mask available, masks will be provided for you at the clinic. You will find hand sanitizer at the entrance in our hand sanitization zone.
