2.1 Our Clinical Services – An overview of Norfolk Primary Care 2.11 Asylum Seeker Service & Vulnerable Adults Service (VAS) I. Asylum Seeker Service
Norfolk Primary Care (NPC) delivers the Asylum Seeker Community Healthcare Team (ASCHT) in collaboration with Norfolk County Council’s People from Abroad Team (PfAT). The service supports asylum seekers residing within the Greater Norwich area to access appropriate health and care services. It provides a coordinated and integrated approach to addressing both the health and wider social needs of asylum seekers, migrants, and refugees. ASCHT operates as part of NPC’s wider Vulnerable Adults Model and contributes to the delivery of Norfolk’s Inclusion Health Service. The team adopts a proactive, case-managed outreach model, designed to respond to the complex and often unmet needs of this population. Support is tailored to individual circumstances and focuses on improving access, engagement, and health outcomes. The multidisciplinary team comprises both clinical and administrative staff, enabling the delivery of coordinated care, effective case management, and strong system navigation support. The ASCHT primarily supports individuals seeking asylum who are: • Residing in contingency hotel accommodation or dispersal housing across Norfolk • Referred via accommodation providers (e.g. SERCO) for an initial health assessment, following registration with a GP practice The service ensures that patients receive timely health assessments, appropriate signposting, and ongoing support, while working closely with primary care and partner organisations to promote continuity of care and reduce health inequalities.
