Governance and Oversight Patient Safety Incidents (PSIs) and associated themes are reviewed monthly through the Patient Safety Incident Response and Learning Group, with bi-monthly reporting to the Quality and Safety Committee. Complaint themes are triangulated alongside incident data and incorporated into the Annual Quality Account, which is formally reviewed and approved by the Board. All incidents are managed in line with PSIRF principles, ensuring a consistent focus on timely resolution, learning, and continuous improvement. The overall PSI rate of 0.22% of total patient contacts provides assurance of a low level of harm relative to service activity. Where relevant, data interpretation has taken account of variation in reporting periods, including a shorter audit period for Millwood Medical Practice (December 2025 to March 2026). Overall, these findings demonstrate that NPC’s patient safety framework aligns with national expectations and supports a culture of transparency and learning. Continued focus on strengthening system resilience, clinical processes, and organisational learning will further enhance patient safety outcomes. Targeted actions will support greater consistency, reliability, and measurable improvements in care quality.
