These insights are being used to inform targeted quality improvement and enhance patient experience across services. Some complaints were assigned multiple themes; therefore, totals exceed the number of complaints (n=168). This reflects the complexity of patient experience and supports a more comprehensive understanding of contributing factors for organisational learning. This approach aligns with best practice in complaints analysis and supports a systems-based methodology, ensuring that learning reflects the full complexity of patient experiences rather than attributing concerns to a single cause.

The systematic use of harm data enables NPC to move beyond incident reporting alone, ensuring that insights are translated into targeted, measurable improvements in care quality and patient safety. By focusing on both actual harm and potential risk, the organisation maintains a proactive, learning-led approach that supports the delivery of safe, effective, and high-quality care. The significant complaints and themes are discussed departmentally at each monthly clinical group meetings and reported centrally to the bi-monthly Quality & Safety Committee. The complaint themes are reported in the Annual Quality Account, which is signed off by the NPC Board.
