Major Issues may include: } “Missing Information”- issues that could be “Serious Problems,” but where not enough information is available to make such a determination. Issues will be paired with recommendations to conduct additional testing, obtain additional documentation, or take whatever action is necessary to determine whether the issue is a “Serious Problem.” } “Serious Problem”- situations where a known condition or practice violates applicable code or deviates from a relevant industry standard, and where such condition or practice, alone or in combination with other known factors, poses a substantial risk of causing serious injury or damage to the environment, public health, or safety. } Unresolved issues identified in a previous EPHS site visit.
