Foreword
Here at Patient Empowerment Network ( PEN ), we value the opportunity to reach so many cancer patients and be able to relate and collaborate with them through curated content that we believe is relevant . Through PEN - Powered Activity Guides such as this one , our # PatientChats , blog posts , and educational media , PEN ’ s volunteers ; who are cancer patients and / or caregivers themselves ; take pride in driving PEN ’ s mission “ to fortify cancer patients and care partners with the knowledge and tools to boost their confidence , put them in control of their healthcare journey , and assist them in receiving the best , most personalized care available to ensure they have the best possible outcome .” Therefore , we have decided to identify ourselves as Empowerment Leads rather than Network Managers . While we do continue to focus on building networks within the cancer patient community , we found that “ leads ” fit better than “ managers ” as we look to lead patients in the right direction when it comes to quality , comprehensive cancer care . Health policy is focused on providing comprehensive care . It can be defined as , “ the laws , regulations , actions , and decisions implemented within society in order to promote wellness and ensure that specific health goals are met .” Examples of healthcare policy include protocols on how healthcare professionals respond to different medical emergencies , the handling of and administering of drugs , and access to healthcare coverage . As you can probably imagine , healthcare policy impacts patients and their families in a myriad of ways . From a cancer patient ’ s perspective , a policy can determine how patients are treated during their time in a hospital or infusion clinic . It can determine what drugs are available to patients and how much they ’ ll cost them and their employers . Perhaps most importantly , it can address the social determinants of health or the barriers that are in place that prevent patients from receiving care . Access to clean water , fresh foods , affordable housing , childcare services , and transportation all play a role in whether or not a patient ’ s care is not only equal to those of other patients but equitable . In our 10th PEN - Powered Activity Guide , you will find information on insurance and treatment access , how “ value ” in health care is defined , tools you can use to advocate for your own health , and more ! Please remember to share with your family , friends , and other patients , and we hope you enjoy it .

Designing Sustainable and HealthY Communities Healthy Community Design was a concept that could break down the barriers between communities and their necessities by establishing convenience to schools , healthcare facilities , and other benefits that should be accessible ( CDC , 2014 ). This was implemented by the Centers for Disease Control and Prevention ( CDC ). While this initiative is no longer funded , the information provided can still serve value when determining how to improve communities .
BENEFITS
Increasing physical activity by placing schools , businesses , and other essentials within walkable distance
Reducing the use of transportation due to decreased distances
Improving community morale by implementing centers and spaces to increase communication
Community and street scale design interventions that improve walking and bicycling infrastructure lead to increases in physical activity . This could reduce the estimated annual medical cost of obesity ($ 147 billion in 2008 U . S . dollars ). 1 2
Developing healthy communities has been demonstrated to play an integral role in its residents ’ lives . These types of communities aid in decreasing the disparities experienced . There is a requirement to understand exactly what the community needs in order to develop a strategy to address these issues . It takes a village to make change , and support in understanding the needs of the community must come from community , local , state , and federal levels . Developing a healthy community is definitely possible . It just takes collaboration , strategy , and planning . Where people live has the ability to affect their overall quality of life . To ease associated stresses , communities can work together to create a more sustainable life .

Moving from Equality to Equity
Health equality means giving everyone the same opportunities , care , and services . This has been the focus for laws , polices , procedures , and medical training . This focuses on elimination of differences in health diagnoses and treatments . Health equity focuses on fundamental justice .
Health equity is achieved when every person has the opportunity to “attain his or her full health potential” and no one is “disadvantaged from achieving this potential because of social position or other socially determined circumstances. 1
In most states where data are available, Black people and American Indian/American Native people are more likely than white people to die early in life from conditions that are treatable with timely access to high-quality health care.

While current cancer screening guidelines focus on equality , data indicates that these need to be adjusted to disparities within specific communities . The incidences of preventable cancer and higher mortality are impacted by the type of cancer , geographic location , race and ethnicity , country of origin , and gender . To address the underlying issues and individual needs , public health needs to move toward an equity - based approach . The goal is to ensure equal access to quality healthcare and good health , even if this requires giving some people more support and resources . Equity - minded medical professionals must consider how social determinants of health such as access to a healthy diet , stress , and trauma affect outcomes . They must also consider how apparently equal systems lead to inequitable and unjust outcomes . An equity lens moves health systems closer to removing disparities . It also encourages policymakers to think about the myriad ways social environments affect health .

Co-pay accumulators
Prescription medications can be super expensive , so it ’ s a good thing that there are assistance programs available to help patients cover some of the costs . Assistance is often provided by charities , nonprofits , or drug manufacturers , and is designed to offset the cost of treatments and medications . The only catch is that there are now other programs in place that seem to negate any of the benefits that the assistance programs provide . If it sounds confusing , that ’ s because it kind of is , but the simplest explanation is this : Pharmaceutical companies , and other organizations , often offer programs to offset the high cost of medications . The programs are known as copay assistance programs , or sometimes coupon programs or co - pay savings programs .
The extra costs often prevent patients from using specialty therapies or following the doctor recommended treatments . STUDIES INDICATE THAT THE RESULT OF COPAY ACCUMULATOR PROGRAMS IS POORER HEALTH OUTCOMES AND POSSIBLE HIGHER COSTS TO THE HEALTHCARE SYSTEM.

Since arriving on the scene , the copay accumulator programs have been controversial . Patient advocacy organizations say the accumulator programs are unfair . Organizations such as the Immune Deficiency Foundation ( IDF ) have noted that it doesn ’ t make sense that the source of the payment for the medication should matter . The best way to find out if a plan contains a copay accumulator or maximizer program is to call the insurance provider ’ s customer service number and ask them directly . Be sure to specifically ask whether or not the plan prohibits third party ( manufacturer , charity , or nonprofit ) contributions from counting toward the deductible and out - of - pocket costs .

What You Need to Know About Non-Medical Switching Now imagine after many months or even years , the patient and doctor have found the correct medication regimen . The patient has improved their quality of life and eased the fears that come with chronic illness . Along comes the patient ’ s insurance company , telling the patient that their medications are no longer covered by their plan . Now the patient and doctor must start over to find new medications that may not be as effective not because their medicines are not working but because of cost . Patients reported a slew of consequences , from an impact on their health to increased health care utilization .

In a survey with 350 physicians , they reported they felt non - medical switching , to some degree :
Increased side - effects ( 54 . 0 %)
Increased medication errors ( 56 . 0 %)
Increased patients ’ out - of - pocket costs (~ 50 %)
Increased medication abandonment ( 60 . 3 %)
Changes to longstanding drug regimens for patients require more doctors ' appointments and lab testing to get the disease symptoms stabilized again . Often , the patient needs to use the services of an emergency room and a hospitalization to get symptoms under control . The newly prescribed medications forced upon the patient and doctor can have new side effects and medical complications . Chronic illness is by definition long - lasting and should be managed by the patient and doctor . When insurance companies cross that boundary and insert themselves into the middle of that relationship , there are steps the patient can take to protect themselves . It is important to find a doctor that is always easily accessible to communicate with .

Value Assessments From Patient and Provider Perspectives
Value assessments are being used by healthcare policymakers to gather information to drive collaborative research and partnerships toward positive changes . It ’ s important to gather information from patients and healthcare providers to ensure that different perspectives are captured for analysis . The factors that are evaluated in clinical trials and the number of factors are key to the success of drug treatments that become approved from clinical trials . Clinical trial designs that fall short in accounting for enough potential patient situations will set themselves up to fail certain patients . Healthcare access is another area of value assessment that can benefit from patient and provider perspectives . By taking action on patient requests , healthcare providers can reach more patients and diagnose patient conditions sooner compared to patients who were previously going unserved or underserved .

There’s an App for That…Or There Should Be:Utilizing Technology for Better Health Outcomes Health literacy has always been a passion project of mine ever since I was diagnosed with cancer . I stand by the notion that plain language and clear communication leads to better health outcomes . However , communicating with our care team isn ’ t always easy . How many of us have gone into an appointment only to leave the office 10 minutes later , wondering what happened and what our copay went to ? Were all of our issues and questions addressed ?
This is where we have to come in as advocates for our own health , and below are a few ways to do this :
Write down a list of questions you want addressed prior to the appointment
Try to focus on one ailment per appointment
If something doesn ’ t make sense , ask for the information to be explained in another way
Ask questions – you ' re the expert of your body and health
How do we keep track of all of this information , though ? There are patient portals that keep track of our appointments and records , but those can often be hard to navigate , and they lack the capability of being able to enter our own information ( i . e . about how we ’ re feeling ). Additionally , different health systems have different portals , leading not only to lost passwords , but a missed opportunity for integrated healthcare . This is essentially senseless for cancer patients who have to keep track of multiple appointments and medications , all while trying to keep afloat in a system that wasn ’ t built for patients and their caregivers . However , there ’ s a role technology can play here . I ’ ve heard of patients carrying around large binders of their records from appointment to appointment . But if we ’ re being honest , I don ’ t believe a physician or other member of a patient ’ s care team is going to take the time to go through it . Instead , utilizing the power of the device that we ’ re constantly carrying around and looking at may be the way to go ( in addition to a smaller folder or journal for those that are comfortable with paper ).
If we think about it , there ’ s an app for everything , and having an app to keep track of our cancer journey should be no different .

What should this app be able to do ? Here are a few things that I think are especially important :
Ability to connect with all members of your care team ( primary care doctor , oncologist , nurse navigator even if they work in different health systems ). 2 Ability to connect with caregivers and share information with them . 3 A diary to describe daily thoughts , symptoms , and side effects , flagging specific keywords that can alert a member of your care team 4 A calendar with appointments ( date / time , office location , directions ) 5 Tips to assist with mental health ( i . e . offering local or national support groups [ both virtual and in - person ], counseling that accepts insurance and / or is offered on a sliding scale ) 6 Medications ( dosage , picture of what it looks like , how to take it , and what to do if you accidentally miss a dose or take more than what is prescribed , ability to refill ) Blood work ( results and what they mean [ featuring a scale of what ’ s low vs . normal vs . high ], what to ask your doctor about in terms of next steps ) Imaging ( results and what they mean , what to ask your doctor in terms of next steps ) Ability to keep track of : 1
Exercise routines with different forms of exercise ( yoga , Pilates , HIIT , weightlifting , walking and running , etc .) based on how you ’ re feeling side effect - and energy - wise 7
Information about nutrition through the different phases of a cancer journey ( pre - treatment vs . in - treatment vs . post - treatment ) that includes recipes 8
Most importantly , all of this information should be in plain language that ’ s easy to understand in whatever language the patient is most comfortable reading 9
Having an app that features all of these capabilities , I believe , would push the needle forward in patient care , not only creating better health outcomes , but a more satisfied patient . What would you add to the list ?

Hidden Garlic Broccoli and Beef EATING WELL
INGREDIENTS
1 ¼ lbs flank steak , sliced thin 1 tbs vegetable oil 2 cups broccoli florets 2 tbs minced fresh ginger ¼ cup oyster sauce ¼ cup beef broth or water
1 tbs brown sugar 2 tsp toasted sesame oil 1 tsp soy sauce 1 tsp cornstarch Salt and pepper to taste
RECIPE
1 .
Heat a large pan over medium heat . Add oil and once it is hot , add the broccoli and cook until soft ( about 4 minutes ). In a separate bowl , mix together oyster sauce with water or beef broth . Once combined , add brown sugar , sesame oil , and cornstarch . Set the sauce aside . Add sliced beef and prepared sauce to pan and mix . Salt and pepper to taste Add ginger and garlic to pan and mix . Cover and cook medium low heat until beef is cooked through . To serve , sprinkle with sesame seeds and green onions .

INGREDIENTS
1 tbs olive oil Salt and pepper Juice of 2 lemons
1 tbs fresh thyme leaves 1 tbs fresh chopped parsley leaves Pinch of red pepper flakes
¼ cup shaved Parmesan cheese 2 to 3 cloves of garlic , minced
RECIPE
Preheat oven to 400 ° F . Toss Brussels sprouts in olive oil & sprinkle salt and pepper . Transfer to a parchment lined baking sheet . Place cut side facing down . Bake for 30 minutes . 1 lb pizza dough 4 tsp extra virgin olive oil , plus more for drizzling 2 large leaves Tuscan kale , ribs removed , leaves torn into 1 - inch pieces
Kosher salt 2 links spicy sausage , casings removed 1 ripe tomato , seeded and coarsely chopped 5 fresh basil leaves 2 tbsp crumbled goat cheese
1 clove garlic , minced 2 tbsp chicken broth
RECIPE
Prepare pizza crust : Shape dough into large rectangle . Heat 1 tsp oil in skillet . Add kale and cook until beginning to wilt , 2 minutes . Add garlic and cook , stirring , 1 min . Add broth and cook until liquid has evaporated . Season with salt and transfer to bowl . Add 1 tsp oil to skillet and heat . Add sausage and cook through ; set aside . In food processor , pulse tomato , basil , remaining 2 teaspoon oil and pinch salt until combined but still slightly chunky . Spread tomato sauce over crust and top with kale , sausage , and goat cheese . Bake until goat cheese starts to brown and kale begins to char , 8 to 12 minutes . 1 . 2 . 3 . 4 . 5 . 6 . 7 .
