ADOPT Adopt social determinants of health screening and referral as a standard of care. COLLABORATE Collaborate with community public health partners for preparedness and response to pandemics and disasters. EMPHASIZE Emphasize teen and young adult health outreach and care. ENGAGE Engage in city, county, state and national policy and resource education and advocacy .

EXPLORE Explore expansion of transportation services.

GROW Grow community-based outreach, education and prevention services. INCREASE Increase Learning Community Center partnership and impact.

EXPAND SITES

CONSTRUCT Construct an additional building on the Livestock Exchange Health Campus and plan a clinical location at 98th and Giles.

CREATE Create a facility and finance plan with a 10-year horizon.

DEVELOP Develop a plan and open new pediatric clinics.

ESTABLISH Establish a dental surgical suite and hygiene outreach.

EXPAND Expand clinical hours of service and open a second pharmacy location.

EXPLORE Explore opportunities for additional school-based and rural sites.

LOCATE Locate additional administrative space and grow infrastructure.

OPEN Open and operate pediatric dental clinic at 84th and Q street.

GROW Grow access to specialty care for colonoscopies, mammograms and dental surgery.

IMPLEMENT Implement telehealth on demand.

LAUNCH Launch OneWorld optometry services.

OFFER Oer traditional and new models of behavioral health services. OPERATE Operate part-time clinic at Stephen Center and explore part-time clinic at the Intercultural Senior Center. PROVIDE Provide specialized access and services in multiple languages for diverse and aging populations.
