• Cancer Advisory Committee : Establish a Cancer Advisory Committee to implement, guide, and oversee BBAHC’s cancer plan. • Process Frameworks : Develop basic process frameworks in advance of having medical staff and case managers engage new responsibilities or activities. • Resource Effectiveness : Deploy current resources to be more effective. For example, improve EHR effectiveness and utility in aspects central to cancer prevention and care.
• Screening Completion Rates : Improve cancer screening rates by utilizing Artificial Intelligence (AI) for focused outreach. Conduct pre-visit chart reviews by case managers to inform focused, provider-led conversations during the medical visit. • CHA/P Engagement : Support CHA/P capacity for expanded engagement in population health strategies.
• Cancer Panel : Establish a cancer patient panel with RN case manager to coordinate clinical aspects of cancer care, including diagnostic testing, treatment, and post-treatment follow-up. • Cancer Navigator : Hire a cancer navigator to guide cancer patients and caregivers, including non- beneficiary patients, in treatment logistics (e.g., transportation, qualified escorts, housing, fiscal resources).
