Functional and Esthetic Crown Lengthening

Functional crown-lengthening procedures may be provided in order to facilitate restorative therapy, that is, expose subgingival caries or a subgingival fracture, establish a ferrule length and create a cleansable embrasure space. In the presence of delayed passive eruption, esthetic crown-lengthening procedures can improve esthetics. This course will be review the biologic width and the concept of the restorative ferrule, as well as the indications, contraindications, techniques and post-operative management for functional and esthetic crown-lengthening surgery. Various clinical cases will be presented in order to review flap design, osseous management, suturing techniques and post-operative protocols for periodontal surgery. Post-operative management will be discussed. Learning Objectives: • Review the concept of the biologic width and the restorative concept of the ferrule • Indications and contraindications for functional crown lengthening, and indications and contraindications for esthetic crown lengthening • Review flap design and the principles of osseous resection to achieve tooth exposure

Dr. Timothy Hempton is a Diplomate of the American Board of Periodontology and also maintains a private practice limited to periodontics and dental implants in Dedham, Mass. He is an Adjunct Associate Clinical professor at Tufts University School of Dental Medicine. Dr. Hempton is on the editorial board for the journal Dimensions of Dental Hygiene and contributes to the column for dental hygienists, “Ask the Expert.”
