Crown-lengthening procedures may involve osseous resective therapy in addition to surgical management of soft tissue. These hard- and soft- tissue alterations may be provided in order to obtain a ferrule height and establish a biologic width. In the presence of delayed passive eruption, esthetic crown-lengthening procedures can be provided to improve esthetics. This course will utilize two plastic typodonts to provide a simulation in functional and esthetic crown lengthening. On the first model, the participant will experience a practical exercise in functional crown lengthening by exposing a maxillary premolar with a subgingival fracture. On the second model, the participant will perform an esthetic crown-lengthening procedure on a model of the maxillary anterior sextant presenting with delayed passive eruption. Techniques for incisions, osseous therapy and suturing will be reviewed. Indications and contraindications to surgical exposure will be discussed. Learning Objectives: • Learn indications and contraindications for crown lengthening and how to design, incise and elevate a flap • Learn ostectomy and osteoplasty to achieve tooth exposure for restorative therapy or for esthetic improvement • Learn how to suture an elevated mucogingival flap

Dr. Timothy Hempton is a Diplomate of the American Board of Periodontology and also maintains a private practice limited to periodontics and dental implants in Dedham, Mass. He is an Adjunct Associate Clinical professor at Tufts University School of Dental Medicine. Dr. Hempton is on the editorial board for the journal Dimensions of Dental Hygiene and contributes to the column for dental hygienists, “Ask the Expert.”
