4. Initial letter of appointment including track (department administrative assistant will add this).

5. Personal statement from candidate which includes a description of a meaningful contribution to the department, how they actively participate in education and mentoring, and a description of the individual’s career goals (not to exceed two pages).

6. Copy of the most recent annual evaluation (department administrative assistant will add this).

7. Letters of recommendation (Note: Letters of recommendation cannot be from the Department Chair or an RPT committee member)

a. Reappointment: At least three letters of recommendation from faculty who are in a position to judge the quality of the candidate’s activities are required except for faculty members’ appointment for a one-year term. At least one letter should be from a direct supervisor. b. Promotion from Instructor to Assistant Professor: At least three letters of recommendation from faculty who are in a position to judge the quality of the candidate’s activities are required. At least one letter should be from a direct supervisor. c. Promotion from Assistant to Associate Professor, and Associate Professor to Professor as well as Tenure Review: i. Clinical Investigator Tenure Track, Research Tenure Track, and Education Tenure Track At least three letters of recommendation from individuals outside the University of Cincinnati are required. It is recommended that these letters be from individuals in other scholarly institutions who are nationally or internationally recognized for their scholarly accomplishments in the candidate’s field; more weight is given to letters from individuals who are experts in the field and who are not collaborators or former mentors of the applicant. These letters are in addition to recommendations that may come from former supervisors and/or associates of the candidate. In addition, at least three letters are required from individuals within the university. At least one letter should be from a direct supervisor. ii. Research Track Same as for Tenure Track. iii. Clinical Service Track
