Investigative Report

l Regardless of the source of the information, a TGRA should always conduct its own investigation to verify the facts alleged and acquire further clarifying information l Such investigation should always result in an investigative report, identifying the steps followed and the information collected.

Investigative Report

An Investigative Report consists of a preliminary statement, findings of fact, opinions, recommendations, and evidentiary exhibits. For some types of investigations, exhibits may include verbatim transcripts or summaries of witness testimony. Other documents that are not probative, that do not tend to prove or disprove a fact, may be appended to the report as enclosures.
