5. The Manager taking the information will also complete an accident investigation supplement, which will include pictures and a drawing of the scene if necessary. The accident investigation supplement will be forwarded to the designated insurance representative along with Associate Injury Report Form as mentioned above. 6. When making a Workers Compensation report the Manager will gather the facts from the Associate as well as witnesses and not admit or imply any liability for the accident. 7. Ensure your Associate knows we care about them, their injury and theirrecovery. HEART ATTACK PROCEDURE

Recognize the signals of a possible heart attack. − Severe indigestion. − Muscle and/or joint pain. − Severe chest pain.

• Notify the Hotel Operator by dialing “0” and have the operator call 911.

Convince the victim to stop activity and rest.

Help the victim to rest comfortably.

• Try to obtain information about the victim’s condition.

Comfort the victim.

Monitor the victim’s condition.

• If you are trained and certified, be prepared to give CPR if the victim’s heart stops beating. NOTE: Many heart attack victims delay getting care. Nearly half of them will wait for two hours or more before going to the hospital. Often, they do not realize they are having a heart attack. SHOCK Caring for Apparent shock victim involves the following simple steps: • Notify the Hotel Operator by dialing “0” and have the operator call 911. • Have the victim lie down. • Control any external bleeding. • Help the victim maintain normal body temperature, provide a blanket as needed • Try to reassure the victim and provide comfort
