SHARED PRIVATE OCCUPANCY: $695.00 LARGE PRIVATE OCCUPANCY: $740.00 Medicare does not cover the cost of a private room unless deemed medically necessary. The resident, if choosing a private room, is financially responsible for the difference between what Medicare pays and the private room rate plus the daily Medicare co-insurance rate. Daily Rate Includes: room, wheelchairs/walkers, housekeeping services, social services, nursing services, basic toiletries, linen service/laundry, meals (including special diets), *activities, standard cable service, and local telephone service. *Occasionally, activities that are planned outside of the facility may be charged in addition to the published room rate. Not Included in Daily Fee 1. Oxygen Therapy: concentrator-$1.25/day; high volume concentrator-$3.00/day; Portable Tank- $9.00/tank 2. Specialty Mattresses: Standard: $10.00/day Wide: $17.00/day 3. Isolation/Precaution Supplies: $15/day 4. Beauty/Barber Haircut Men $17.00 Haircut Women $19.00 Shampoo & Set $24.00 Shampoo, set & cut $38.00 Permanent w/ Shampoo, Cut & Set $70.00 Hair color, Shampoo, & Set $57.00 Manicures $15.00 Wax $ 9.00 Wax Lip, Chin, & Eyebrow $13.00 5. Medical, dental, and eye appointments requiring St. Anne ’ s transport services will be billed based on the destination zone and the type of transport provided. Please see reverse side for rates. 6. There will be a $35/hour charge if an employee is needed to be in attendance during medical appointments. If a family member wishes to accompany a resident to a medical appointment, please contact the Transportation Office at 285-6125. 7. Use of van for non St. Anne ’ s Retirement Community sponsored activities will be determined at time of request. Other Items Not Included in the Daily Rate 1. Physician fees, Podiatrist fees, Dental/Eye Care 2. Medications 3. Lab/x-ray fees 4. Stationery supplies 5. Dry cleaning 6. Postage 7. Photocopies (Medical/Financial Records) 8. Special Skin Care items Note: Rate for these services must be set to cover costs and are subject to change. Medicare A Residents : daily rate includes room, board, and all medical care. It does not include long distance phone charges, photocopies, stationary supplies, dry cleaning, postage, or fees to participate in special activities. Medicare will not pay for bed holds. Medical Assistance Residents : daily rates include room, board, medical care, and one haircut per month. It does not include long distance phone charges, photocopies, dry cleaning, postage, additional beauty shop charges, or fees to participate in special activities. Medical Assistance will pay for medical bed holds up to 15 consecutive days and 30 days per year for personal leave.
