CAP AUDIT FINDINGS Staff Training
COMMON CAP AUDIT FINDINGS As noted in the introduction, insufficient training is a common finding identified by the Justice Center during the CAP audit process. Some of the most common reasons for an insufficient finding include: 1. Little-to-no documentation is provided to confirm training or retraining took place
2. Documentation was provided but was incomplete or lacked necessary information
3. Training was completed but only for a portion of staff (ex: part- time or per-diem staff were not included)
4. Training was conducted virtually and no attendance list was captured
5. Training was completed, but not in a timely manner following the incident that prompted the training
6. Initial training documentation was submitted as verification for training, but the corrective action required an updated or new training was needed following an incident
7. The training was not relevant to the issue identified
8. Training was completed on outdated plans of care or incorrect information
9. Training was not provided immediately upon return to work, for staff who were absent from a required training
10. Training was conducted by staff who did not have the proper credentials, experience, or education to provide it
