FROM OUR CEO OneWorld has weathered many storms in its history. Since our founding more than 55 years ago, we have emerged from each storm stronger and with a deepened commitment to our mission to Care for All People.

Today’s environment presents many tests of our strength and determination. From changes in government funding to Medicaid, and attitudes about immigration to shifts in the economy, OneWorld continues to weather storms. Yet our mission guides us like a lighthouse beacon. At OneWorld, we understand that daily acts of care and kindness add up to something even mightier than stormy seas. Every person who walks through our doors matters, a truth illuminated by the words and actions of our team members. Each of our 51,643 patients counts on us to live our mission and values through the provision of high quality, compassionate health care and supportive services. Even as our patients face uncertainty, our mission drives us to ensure each person is seen, heard, valued and cared for – and their family members are reassured by the care they receive. In this annual report, we reflect on the achievements of 2025 in three key areas: enhancing access to quality health care, workforce development and strategic planning. You will also find information about our patient demographics, service offerings and financials. We share these achievements to shine a light on the wins our OneWorld team has made possible. Together, we also look ahead with a bold vision – thanks to the insight of board members, community members and staff that guided the creation of a new strategic plan. We remain committed to reducing geographical, financial and other barriers to care. In 2025, we grew our physical presence and services. This included establishing our first School-Based Health Center (SBHC) to serve Westside Community Schools, as our other School-Based Health Centers offered additional behavioral health. Meanwhile, the Learning Community Center of South Omaha opened a satellite at Bancroft Elementary to serve more families. The pediatric dental clinic completed more treatment plans than ever before and extended hours – thanks to a new dentist, hygienist and support staff. Our dental visits at all locations reached an all-time high. Our Stephen Center clinic grew women’s health services, and a partnership with Creighton University allowed staff to offer pediatric wellness days. The OneWorld Pharmacy noted steady growth in its Med Sync and Unit Dose Packaging program, and advocacy for the 340B program protected patient access to lower-cost drugs. Finally, we grew digital access to health services. Since its launch in 2023, the OneWorld Patient Portal has gained 11,595 enrollments. BEACON FOR HEALTH CARE ACCESS

In spring 2025, OneWorld broke ground on a new building that will allow us to grow our workforce development programming, expand behavioral health services and offer early child development services. The future OneWorld Health Care Workforce Development Center joins the trio of buildings that now make up the Livestock Exchange Campus in South Omaha. Construction on the three-story, 29,000-square-foot building continued throughout the year, with completion expected in late 2026. BUILDING HOPE AND OPPORTUNITY

The Health Care Workforce Training Center will allow OneWorld fill service gaps, train our workforce and create a pipeline to health care jobs in our community. The project is made possible thanks to the support of the Nebraska Department of Economic Development, the MacKenzie Scott Foundation, other foundations and private donors.

Success in workforce development reveals itself in individual growth, organizational strength, improved service delivery, and economic impact for our community. OneWorld launched two new leadership development initiatives: the Emerging Leaders Employee Resource Group and a Mentorship program, while our Leadership Academy graduated eight. Such leadership programs increase employee engagement, strengthen our organization and support succession planning. Existing efforts led to advanced training for more employees. Our Pathways Program helped 10 medical assistants earn their Certified Clinical Medical Assistant (CCMA) certification, with more in the pipeline. We also invested in employee engagement and appreciation programs, such as the inaugural OneWorld Values Awards, which helped celebrate some of our most values-aligned team members. Employee retention rose from 75% to 82% from 2024 to 2025. Across that same timeframe, our leadership bench strengthened with the creation of new associate director roles. SPOTLIGHTING OUR WORKFORCE

ILLUMINATING THE PATH

Clear strategy shows us where we are and illuminates a path forward. Last year, OneWorld completed action steps for the 2023-2026 strategic plan – and created a new plan to guide us through 2030. Under the concluding strategic plan, OneWorld celebrated achievements relating to community wellness, site expansions, providing exceptional holistic care and services, offering easier access to care, and employee wellbeing. These achievements were notable, yet more work lies ahead as OneWorld strives to close service gaps in our community and make quality health care and support services accessible to all. Together, OneWorld’s staff, board members and community stakeholders set a bold strategic aim for 2026-2030: to position OneWorld as the region’s best primary health care organization dedicated to comprehensive Care for All People. The plan includes four strategic objectives, each with its own set of tactics. The objectives are based on the community health needs assessment, patient and employee insights, and a changing external environment. These objectives are to: • Strengthen our capacity to meet growing community needs • Harness technology to enhance the patient and employee experience • Enhance the perception of OneWorld across the metropolitan area • Expand our array of services to facilitate access to comprehensive primary care With a robust strategic plan guiding our work, OneWorld remains dedicated to carrying our mission forward far into the future.

OUR MISSION IN ACTION For a patient with a history of unstable blood pressure, evenings had become a source of dread. Each night followed a pattern: a headache would set in, her blood pressure elevated, and her anxiety spirals would lead her to the emergency room. Part of the problem was her medications. She couldn’t always remember whether she had taken them. To help her keep track, the OneWorld Pharmacy enrolled her in the PASS Program – grouping her medications into pouches according to a personalized dosing schedule. With support from a family member and the pharmacy, she shifted to the new system. Less than a year after enrolling in PASS, she made her last ER visit for a blood-pressure related issue. Her normal blood pressure signaled a milestone. Quality, personalized health care makes a human impact – and a likely financial benefit for her and the health system.
