The HVS operates Monday to Friday between 09:00 hours and 17:00 hours, working in close collaboration with GP practices to prioritise and respond to patient needs. This partnership approach ensures that patients receive care in the most appropriate setting, reducing the need for unnecessary travel and supporting equity of access for those who are unable to attend appointments.

During the 2025/26 period, the HVS team completed 7,663 home visits, demonstrating a significant level of activity and demand. Through effective clinical triage and timely home- based interventions, the service has facilitated early identification and management of health issues, helping to prevent deterioration and reduce the risk of crisis presentations.

Clinical assessments undertaken by the service consider not only immediate medical needs, but also functional and social factors, enabling a comprehensive and person-centred approach to care. By initiating treatment within the community, coordinating follow-up with GP practices, and linking with wider community and social care services where required, the HVS plays a key role in supporting continuity of care. This integrated model has contributed to a reduction in avoidable hospital attendances and admissions, helping to ease pressure on acute services while improving patient outcomes and experience.
