2.14 Home Visiting Service
The NPC Home Visiting Service (HVS) has been in operation since April 2024 and continues to support 17 practices across three central Norwich Primary Care Networks (PCNs). The service is directly commissioned by these practices to deliver essential same-day home visits for housebound patients, ensuring that those most vulnerable can access timely care in their own homes. In addition to urgent care provision, the service also supports aspects of long-term condition management, providing a more holistic and coordinated approach to patient care.
