Section 4: Our Key Quality Improvement Priorities The Quality & Safety Committee and the Board of Directors have identified three key domains that underpin our approach to quality improvement, which are: Patient Safety - Ensuring patients are protected from avoidable harm Clinical Effectiveness - Delivering care that achieves the best possible clinical outcomes Patient Experience - Understanding and improving how patients perceive the care they receive These priorities are aligned with the Care Quality Commission’s (CQC) Single Assessment Framework, which evaluates services against five key questions: whether they are safe, effective, caring, responsive, and well-led . Our annual quality priorities are informed by a robust, evidence-based process, drawing on staff and patient feedback, stakeholder engagement, audit outcomes, and service evaluations. This approach ensures that our objectives reflect both the needs of the communities we serve and the expectations placed on high-performing healthcare providers. Our Patient Safety Statement (May 2025), alongside this 2025/26 Annual Quality Report , outlines our strategic objectives and expectations across all areas of the quality agenda.
