organisational culture has further developed towards one of transparency, openness, and continuous learning, supported by regular reviews of safety processes and patient care practices. Significant progress has also been made in workforce development through enhanced engagement and communications to our teams, equipping staff with the skills and knowledge required to deliver safe, high-quality, and compassionate care.

This was demonstrated through:

 The full implementation of the Patient Safety Incident Response Framework (PSIRF), embedding a systems-based approach to learning from incidents.  Adoption of the NHS England ‘Being Fair’ tool to support a just and learning culture.  Delivery and uptake of Level 1 and Level 2 Patient Safety Syllabus training, with this becoming a core expectation for relevant staff groups.  Completion of PSIRF e-learning across the workforce to reinforce learning in practice.  Ongoing engagement with stakeholders and member practices to promote consistent application of PSIRF principles across services.

2. Clinical Effectiveness

Aligned with CQC domains: Effective, Safe, Caring

During 2025/26, we made measurable progress in strengthening clinical effectiveness and improving patient outcomes through the consistent application of evidence-based practice. We continued to embed NICE guidance and best practice standards across services, supported by participation in local clinical audits and clinical pathway improvements. Work was undertaken to improve the efficiency and accessibility of audit processes, enabling staff to complete audits more effectively while maintaining service delivery. We also placed a strong emphasis on supporting the wellbeing and professional development of our clinical workforce.

Key achievements included:

 Increased participation in clinical audits, alongside improvements to audit processes to reduce administrative burden.  Significant progress towards the goal of ensuring 100% of clinical staff receive structured clinical supervision, supported by accessible tools and guidance.
