Overall, the progress made during 2025/26 demonstrates our continued commitment to improving quality across all domains, supported by strong leadership, workforce development, and meaningful engagement with patients, staff, and partners. 4.2 CQC “We” Statements – Evidence of Quality (2025/26) Safe We have embedded a consistent and mature Patient Safety Incident Response Framework (PSIRF) across all central services, ensuring a proportionate, learning-focused response to incidents. We ensure all staff understand and apply a just and learning culture, supported by the NHS ‘Being Fair’ framework, promoting openness and psychological safety. We have standardised incident reporting, investigation, and shared learning processes across member practices to reduce variation and improve safety outcomes. We proactively identify and mitigate risks through the triangulation of incident data, complaints, prescribing data, and audit findings. We have strengthened safeguarding arrangements, ensuring robust processes are in place to protect vulnerable adults and children in primary care settings. We work collaboratively with system partners across the ICS to ensure safe transitions and delivery of care and shared learning from safety events.
Effective
We consistently deliver evidence-based care, supported by the implementation of NICE guidance and local clinical pathways across all services. We use population health management (PHM) data to target interventions, improve outcomes, and address health inequalities. We undertake regular clinical audits and quality improvement initiatives, with clear links to measurable improvements in patient outcomes. We aim for all clinical staff have access to structured clinical supervision and protected learning time, supporting continuous professional development. We support multidisciplinary team (MDT) working, enabling patients to receive care from the most appropriate professional at the right time.
