WHO WE ARE Since 1997, Northern Nevada HOPES (HOPES) has been on the front lines of community health, caring for those with the fewest resources and solving community problems in unexpected ways. Over 27 years, we have evolved to meet the changing needs of our community, offering comprehensive, holistic healthcare that addresses some of our region’s most pressing challenges.

Our generous donors and supporters enable HOPES to continue offering high-quality healthcare that changes lives. In 2023, we were honored to be handpicked to receive an unsolicited $6 million gift from MacKenzie Scott. MacKenzie Scott’s team can give to anyone they choose, and they chose HOPES, recognizing the tremendous positive impact we have on our community. It was both completely unexpected and tremendously meaningful. We are grateful and proud to receive such a generous donation that will help HOPES continue to transform how it provides for the needs of our community, and advance healthcare in our region.

WHO WE SERVE Since the beginning, HOPES has served those in our community with the greatest needs and the fewest resources. Today, we are a choice for these patients, as well as many others who have a greater range of healthcare options. We meet our patients where they are, offering dignified, inclusive, high-quality healthcare for all community members. Of the 14,000+ patients we served in 2023:

37% live below 138% of the Federal Poverty Level (An individual earning less than $14,657 per year)

56% live below 200% of the Federal Poverty Level (An individual earning less than $25,760 per year)

10% are experiencing homelessness (22% of services are utilized by patients experiencing homelessness)

43% are on Medicaid or Medicare

32% are uninsured

Approximately 22% of individuals living in HOPES’ service areas are living in poverty, compared to 11% in Washoe County.

TRANSFORMING COMMUNITY HEALTH THROUGH PATIENT-CENTERED CARE At HOPES, we believe that high-quality primary care is each patient’s pathway to living a healthier life, allowing them to contribute to a healthier community. Our services are designed to provide clients with holistic and patient-centered care that addresses the psychological and social concerns affecting their ability to access, engage with, and adhere to healthcare treatment. Our Model of Care A central component of the HOPES care model is treatment teams, where all providers involved in patient care work together to address the patient’s unmet medical, behavioral health, socioeconomic, and social support needs. Primary care for all All members of our community deserve access to high-quality healthcare. This is a defining belief of HOPES. Addressing healthcare access barriers is crucial to our ability to transform health in our community. For the uninsured, and those unable to pay, all of HOPES’ services are provided on a sliding-fee scale, allowing us to guarantee care for the most disenfranchised members of our community. We also provide support services that help ensure better health, including case management, substance use treatment, nutrition education and food prescriptions, insurance benefits counseling and enrollment, medical transportation, and connection to medical-legal counseling. Our unique, welcoming healthcare model ensures high-quality care for medically underserved populations, such as those experiencing homelessness, those suffering from mental health concerns, and those living in poverty.

By addressing a wide range of health concerns and removing barriers to healthcare, HOPES is improving the health of our community, one patient at a time.

Our Impact By caring for underserved populations, HOPES contributes to statewide efforts to improve overall health in Nevada. Our work helps reduce emergency room visits, reducing the financial burden area hospitals face from serving the region’s uninsured population. Taxpayer dollars are also saved by providing integrated case management and behavioral health services, which reduce homelessness, incarceration rates, and reliance on public benefits.

TRANSFORMING OUR COMMUNITY THROUGH CHRONIC DISEASE MANAGEMENT

A Holistic Approach to Managing Diabetes Diabetes is a serious chronic disease that can lead to heart disease, stroke, amputation and death. One of the most common causes of diabetes is obesity, and as of 2023, 27% of adults in Washoe County were characterized as obese. At HOPES, managing diabetes effectively means providing holistic care that includes nutrition resources and patient education. Our team of dedicated Community Health Workers help patients navigate their diabetes care journey, engaging with them to understand their unique needs and working together on a path toward wellness.

Education is Healthcare We recognize that when it comes to managing diabetes effectively, patients are more successful when they are informed and empowered to make healthy choices. Providing education through our diabetes education classes and one-on-one nutritional counseling sessions helps ensure a sustainable path to diabetes management. By offering tailored guidance, emphasizing lifestyle changes, and providing diverse tools for effective sugar level management, we’re empowering individuals to make informed choices about their health while building a supportive community with others facing similar challenges. A Prescription for Healthier Food Options Food insecurity can have a serious impact on physical and psychological health. For patients experiencing food insecurity, we provide a food prescription, which allows patients to access healthier food options from various community food pantries on a weekly basis. By removing barriers to a nutritious and balanced diet, we’re able to improve the overall health of our community.

TRANSFORMING OUR COMMUNITY THROUGH INCLUSIVITY At HOPES, we believe that all people deserve access to care. As a safe and accepting place for all, we provide culturally competent, judgment-free healthcare to lesbian, gay, bisexual, transgender and queer/questioning individuals. We are a proud ally of the LGBTQ+ community and actively

show our support through community outreach and education. Members of the LGBTQ+ community can expect high-quality, compassionate care, including:

TRANSFORMING OUR COMMUNITY THROUGH HARM REDUCTION

OUR MISSION Provide compassionate, comprehensive treatment to inspire hope and enhance quality of life, through the process of recovery.

Substance Use Treatment We believe that a sober life starts with meeting our patients wherever they are in their recovery journey. By addressing their unmet psychological and socioeconomic needs, we can get to the root causes of their substance use and provide whole-person, dignified care. Our approach to substance use treatment includes Medication-Assisted Treatment (MAT), Peer Support Specialists, and high-quality, compassionate wrap-around health care services. Medication-Assisted Treatment MAT is a cornerstone of our commitment to recovery. Through our MAT Program, we offer a holistic approach to recovery, addressing both the physical and psychological aspects of addiction. This includes counseling and therapy, as well as medication management, which helps reduce cravings and withdrawal symptoms from alcohol and opioid addiction. Peer Support Specialists A fundamental part of our approach to recovery is the integration of Peer Support Specialists — non-clinical employees whose shared life experiences enable them to connect with patients. These specialists meet patients where they are in their recovery journey, offering empathy and stability for those who need it most. Peer Support Specialists are active in all aspects of patient care, including behavioral health, MAT and residential aid at Hope Springs, serving as a resource and connecting patients to other community services that support general wellness.

Change Point While harm reduction is often associated with substance use, it now has a wider range of applications, including sex, diet, health, and many other aspects of self-care. Change Point is HOPES’ Harm Reduction program and is open to all members of the community. It includes the Syringe Services Program, free fentanyl test strips and Narcan kits, and sexual health services, including walk-in STI clinics, PrEP and PEP clinics. Another benefit of Change Point is that it allows us to connect with individuals engaged in substance use and provide them with access to our MAT program and other support services.

Combined with coordinated, personalized care, HOPES’ MAT and Change Point programs profoundly impact community

health, reducing incidence of overdose and connecting individuals experiencing substance use disorders to care and services.

In 2023, there were 1,690 unsheltered individuals identified in the downtown Reno area, a number that has increased by 60% since 2018.

TRANSFORMING OUR COMMUNITY THROUGH HOUSING SOLUTIONS We recognize that improving health outcomes for those experiencing homelessness can only be achieved by addressing their housing needs first. In 2021, we created Hope Springs, the only bridge housing community in our area and a first-of-its-kind solution to our area’s housing and homelessness crisis. At Hope Springs, residents develop and work through individualized care plans, while accessing HOPES’ wrap-around medical and wellness services. With a focus on behavioral health, the program empowers individuals to work through past trauma while building a path toward sustainable, independent living. Through safe housing, behavioral health and medical care, case management, and wrap-around services, Hope Springs residents are able to achieve housing stability, effectively supporting their health outcomes.

TRANSFORMING OUR COMMUNITY THROUGH BEHAVIORAL HEALTHCARE

A healthy community starts with healthy people, and healthy people start with healthy minds.

In the 2022 State of Mental Health in America, Nevada ranked 51st overall for mental health, indicating a higher prevalence of mental illness and lower rates of access to care than anywhere else in the country. By providing comprehensive behavioral healthcare starting at age three, HOPES is tackling the mental health crisis in our community – supporting patients in crisis and providing early intervention for our community’s children.

HOPES behavioral health services are patient-centered, collaborative, and trauma-informed. They include:

• Individual, couple, and family therapy • Adult, child, and adolescent therapy • Play therapy • Adult and pediatric psychiatry • Trauma counseling • Support groups • Psychiatric medication • Drug, alcohol, and gambling treatment

Intensive Outpatient Program HOPES also offers an Intensive Outpatient Program (IOP), which we opened in 2022 to address our community’s need for improved access to behavioral healthcare. HOPES’ IOP is the first of its kind in northern Nevada to provide in-depth and comprehensive mental healthcare to those with Medicaid or no insurance. This program helps patients address emotional issues, substance use, and trauma. Providing a level of care between traditional outpatient therapy and inpatient hospitalization, IOP enables patients to resume their daily lives more efficiently, and with improved mental health.

In a heartfelt act of generosity, an anonymous donor contributed $5 million to the East 4th Street Clinic campaign. The donor requested we name the new clinic after Jerry Smith, a cherished friend and dedicated supporter of HOPES, who passed away in March 2023. Serving as the President, Foundation Manager, and Director of the Nell J. Redfield Foundation, Jerry cared deeply for our community and was passionate about HOPES’ mission. Why We are Expanding HOPES’ current clinic has reached capacity, putting a limit on how many we are able to serve. Additionally, there is a severe lack of behavioral health services in Washoe County, particularly for those on Medicaid or with no insurance. To meet these growing needs, HOPES is building a new clinic on East 4th St., in the heart of one of northern Nevada’s most disenfranchised communities. The clinic, adjacent to the Washoe County-operated Cares Campus (our community’s homeless shelter) and across from Hope Springs (HOPES’ bridge housing community), will offer comprehensive medical and behavioral healthcare services, including Medication-Assisted Treatment, an Intensive Outpatient Program, and an on-site pharmacy. The new clinic will also offer same-day appointments.

HOPES’ new East 4th Street Clinic, The Jerry Smith Community Wellness Center, will increase access to medical and behavioral healthcare services for more than 12,000 of our community’s most vulnerable members.

What This Means for Our Community The neighborhood surrounding East 4th St. is home to mainly low-income residents who lack nearby healthcare services. Current data shows only 12 % of Medicaid-eligible individuals in this area are accessing care. The new clinic presents an opportunity to improve access to healthcare for these individuals.

Financial Impact Currently, when seeking medical care, the majority of uninsured and underinsured individuals in the area are using area hospital emergency rooms or emergency services, significantly increasing costs for taxpayers and overburdening the healthcare system. HOPES’ new clinic will expand services to these underserved populations, saving taxpayer dollars and reducing the burden on local hospitals.

Nationally, community health centers like HOPES reduce overall medical expenditures by 24% compared to non-health center providers. Health centers in Nevada created 2,739 jobs and generated $449,671,335 in total economic activity in 2021. So, supporting HOPES not only improves healthcare accessibility, it contributes to the overall well-being and financial stability of our community.

LETTER FROM THE CEO

2023 was a year of tremendous change for Northern Nevada HOPES, change that accelerated our ability to transform the health of our community. And we couldn’t be more excited. In summer 2023, we received a $6 million gift from MacKenzie Scott. We didn’t apply for this gift, so we were blown away to receive such a generous donation based on the work we do and the impact we have on northern Nevada. This gift permits us to take the next step in filling the healthcare gap in our community. We began building a new clinic on East 4th Street, The Jerry Smith Community Wellness Center, which will double our capacity to serve our community (you can read more about the clinic on page 13) . This new clinic reflects HOPES’ ongoing commitment to meet our patients where they are on their healthcare journey — in the most literal and compassionate way. This location, just west of the highway overpass near Hope Springs, our bridge housing community, is where many of our houseless neighbors find themselves. We are there because they are there. And we are there because of you. When I came to HOPES over a decade ago, it was a small clinic with 600 patients and 26 employees. Ultimately, we will have the ability to serve more than 26,000 community members with a passionate and dedicated team of more than 300. And that gives me great hope for our future.
