Dental implants have been successfully implemented as a routine therapeutic approach in partially and fully edentulous patients. Yet the incidence and prevalence of peri-implantitis is quoted as high as 50 per cent by some experts. Recognition of the fact that it is not the “implant” that fails and then fails, but rather the patient’s bone and soft tissue support which “fails” the implant. The very factors leading to this must be investigated and discussed. Implant “integration” and surgical success can be quoted as 97 per cent, but true success is measured after years of function, that which can mimic esthetic and functional success as blending in with the natural dentition that Mother Nature gave the patient is truly “implant success.” The path to implant replacement begins right at the time of tooth extraction, and losing a body part is not “atraumatic.” The presence of the buccal plate dictates the decision tree, which we will follow. Surgical extraction techniques to predictably maintain the bundle bone, efficacious socket management and predictable soft tissue management will be discussed. Learning Objectives:

Dr. Preety Desai received her board certification as a periodontist in 1995 and completed her specialty M.Sc. degree in laser dentistry in 2016 from Aachen University Germany. Dr. Desai has implemented the erbium all-tissue laser into all aspects of periodontal and implant surgery since 2005. She is a Clinical Associate Instructor at the University of British Columbia, has lectured at AAP and the AO and has also lectured locally and internationally.
