Fax or mail your registration by downloading the registration form from the website at asm.oda.ca . Important: to receive confirmation of your registration and an attendance verification, you must provide an email address for each registrant on the registration form. Complete the form to receive your letter of invitation. This invitation letter does not in any way financially obligate the ODA. Important: the ODA does not assist attendees with visa applications. That is the sole responsibility of the applicant. Cancellation/Refund Policy Requests for cancellations and refunds for conference registrations and paid courses had to be submitted in writing to the ODA by March 30, 2022. Refund requests received after this date or refunds of less than $50 will not be granted. Unusual requests for refunds will be reviewed on a case-by-case basis and will be at the discretion of the ODA. Requests for refunds must include the badge(s) and tickets returned to the ODA. No refunds will be granted unless the badge(s) and tickets are returned at the Debit Desk located on-site in the registration area, 600 Level. A $50 administration fee will be applied to all refunds. Refunds will be processed by May 30, 2022. Please note that no refunds will be issued on-site at the ASM.
